摘要
目的探讨中西医结合治疗方案治疗耐多药肺结核的临床疗效。方法 60例耐多药肺结核病随机分为中西医结合治疗组32例(观察组)和单纯西药治疗组28例(对照组),中西医结合治疗组采用6Z-Lfx-Am(Cm)-PAS-E(Pto)/18Z-Lfx-PAS-E(Pto)化疗方案+参蛤散,单纯西药组采用6Z-Lfx-Am(Cm)-PAS-E(Pto)/18Z-Lfx-PAS-E(Pto)化疗方案。结果两组病例在治疗6个月后,中西医结合组痰菌阴转率和中医证候有效率明显高于单纯西药组,分别与对照比较差异均有统计学意义(P<0.05);在治疗9个月后中西医结合组病灶吸收有效率显著高于单纯西药组,两组比较差异有统计学意义(P<0.05);在治疗12个月后中西医结合组空洞闭合率显著高于单纯西药组,两组比较差异有统计学意义(P<0.05);中西医结合组的药物不良反应率与单纯西药组比较差异无统计学意义(P>0.05)。结论参蛤散辅助西药治疗耐多药肺结核具有较好的效果,有助于促进改善症状,痰菌转阴,使病灶空洞早期闭合,是一种有效的临床治疗方案,值得推广和使用。
Objective Traditional Chinese combined western medicine treatment of the clinical effect of multi-drug resistant tuberculosis（MDR-PTB）.Methods 156 cases with MDR-PTB were selected and randomized into two groups,32 cases in Traditional Chinese combined western medicine treatment regimens group treated by 6Z-Lfx-Am（Cm）-PAS-E（Pto）＋Traditional chinese drug＋18Z-Lfx-PAS-E（Pto）,28 cases in western medicine of standardized treatment regimens group treated by 6Z-Lfx-Am（Cm）-PAS-E（Pto）＋18Z-Lfx-PAS-E（Pto）.Results The two groups were the end of treatment,sputum conversion rate of the traditional Chinese combined western medicine group was significantly higher,the difference was statistically significant（P0.05）.Traditional Chinese combined western medicine group lesions significantly higher absorption rate,the difference was statistically significant（P 0.05）.Traditional Chinese combined western medicine group empty negative rate was higher,there was significant difference（P 0.05）.Traditional Chinese combined western medicine group and control group recurrence rate was significantly（P0.05）.Traditional Chinese combined western medicine group the rate of adverse drug reactions compared with the control group showed no significant difference（P0.05）.Conclusion Participation clams scattered secondary western medicine treatment of multidrug-resistant tuberculosis with good results,and help to promote sputum negative lesions empty early closure is an effective clinical treatment program,worthy of promotion and use.
