PVP治疗老年骨质疏松性椎体非压缩性骨折的疗效观察 被引量:13

摘要 [目的]提出老年骨质疏松性椎体非压缩性骨折概念的初步设想,探讨经皮椎体成形术(PVP)治疗老年骨质疏松性椎体非压缩性骨折的疗效。[方法]选取老年骨质疏松性椎体非压缩性骨折患者57例(共74节),根据治疗方式分为两组,观察组31例(42节)给予PVP治疗,对照组26例(32节)给予保守治疗。采用下腰痛疗效结果评分表LBOS评估疼痛程度和生活质量,同时随访观察病椎椎体形态学变化。[结果]观察组42节椎体穿刺全部成功,无严重并发症发生。观察组31例患者比较治疗前后疼痛程度和生活质量,有显著性差异(P<0.05)。治疗后1周,1、6、12个月患者疼痛程度和生活质量观察组和对照组比较,均有显著性差异(P<0.05)。治疗后1周,1、6个月两组患者病椎高度丢失比较,无显著性差异(P>0.05),治疗后12个月对照组病椎高度丢失显著高于观察组,有显著性差异(P<0.05)。[结论]PVP治疗老年骨质疏松性椎体非压缩性骨折,能迅速缓解疼痛,提高患者生活质量,显著预防椎体的塌陷,防止脊柱畸形的发生。 [ Objective J To propose the concept of vertebral osteoprotic non-compression fracture and explore the efficacy of percutaneous vertebroplasty(PVP) in treating vertebral osteoporotic non-compression fracture. [ Method] A total of 57 patients (74 vertebrae) with vertebral osteoporotic non-compression fracture were divided into the observation group(31 patients ,42 ver- tebrae) and the control group(26 patients,32 vertebrae). The patients in the observation group were treated by PVP and the ones in the control group were treated by conservative therapy. Degrees of pain and life quality were evaluated via Low-Back Outcome Scale(LBOS). And the morphological changes of the vertebrae were observed at the same time. [ Result] Puncture was successfully performed in all patents of the observation group without serious complications. Pain degrees and life quality of the postoperative were significantly different compared with the preoperation( P 〈 O. 05 ). Pain degrees and life quality were signifi- cantly different between the two groups at 1 week, 1 month,6 months, and 12 months after treatment (P 〈 0.05 ). The height loss of the vertebral body showed no significant difference between two groups at 1 week, 1 month and 6 months after treatment(P 〉 0.05) , but the height loss of the vertebral body in the control group was more serious than that in the observation group(P 〈 0. 05). [ Conclusion] PVP is effective in treating vertebral osteoporotic non-compression fracture, which can quickly reduce the pain and improve life quality of the patients. PYP can also effectively prevent the collapse and deformation of the vertebrae.
