授权教育对接受胰岛素治疗糖尿病患者自我管理行为的影响 被引量:17

Effect of empowerment education on self-management behavior of diabetic patients with insulin therapy

摘要 目的探讨授权教育对接受胰岛素治疗的2型糖尿病患者自我管理行为及治疗达标的影响。方法运用授权理论对50例接受胰岛素治疗的2型糖尿病患者进行教育,评估授权教育前及授权教育6个月后授权教育对患者自我管理行为及治疗达标的影响。结果与授权教育前相比,授权教育6个月后患者自我管理行为有明显改善[(75.04±14.88)比(93.42±10.29)分],差异有统计学意义(t=7.157,P〈0.01)。自我管理行为各维度较干预前改善,前后比较差异均有统计学意义,其中饮食控制[(16.10±5.01)比(23.48±4.24)分,t=8.596,P〈0.01],规律锻炼[(11.30±4.94)比(16.52±4.01)分,t=6.076,P〈0.01],遵嘱用药[(11.78±2.41)比(13.92±1.68)分,t=5.270,P〈0.01],血糖监测[(9.44±3.54)比(16.12±3.11)分,t=10.057,P〈0.01],足部护理预防[(15.64±3.18)比(20.36±3.60)分,t=6.689,P〈0.01],处理高低血糖[(10.78±3.41)比(17.24±2.60)分,t=10.329,P〈0.01]。干预前患者的糖化血红蛋白(HbAlC)为(9.67±1.36)%,干预后3个月降低至(7.83±1.02)%,差异有统计学意义(t=7.831,P〈0.01);干预后6个月HbAlc为(8.70±1.67)%,干预后6个月HbAlc与干预后3个月相比,差异有统计学意义(t=8.70,P〈0.01);干预后6个月HbAlc与干预前相比,差异有统计学意义(t=9.67,P〈0.01)。结论授权教育能够提高糖尿病患者自我管理能力,促进其行为改变,从而有利于控制达标。 Objective To evaluate the effect of empowerment education on self-management behavior and treatment efficiency in type 2 diabetic patients with insulin therapy. Methods Empowerment education was implemented on type 2 diabetes patients with insulin therapy, self-management behavior and treatment efficiency were assessed before and after 6 months of education. Results Score of self-management behavior was significantly improved after 6 months empowerment education compared with before [ ( 93.42 ±10. 29 ) vs (75.04 ± 14.88) ] , and the difference was statistically significant ( t = 7. 157,P 〈 0.01 ). Every dimension of self-management behavior had also improved significantly after training, including diet control [ ( 23.48 ± 4.24) vs (16.10±5.01), t =8.596, P〈0.01] , regular-exercise [ (16.52±4.01) vs (11.30±4.94), t =6.076, P 〈0.01 ], medical compliance [ ( 13.92 ± 1.68) vs ( 11.78±2.41 ), t =5. 270, P 〈0.01 ], self-monitoring of blood glucose [ ( 16.12 ±3.11 ) vs (9.44 ±3.54), t = 10. 057, P 〈0.01 ) 1, foot-care [ (20.36±3.60) vs ( 15.64 ± 3.18), t = 6. 689, P 〈 0.01 ] , and correct-treatment of hyperglycemia or hypoglycemia [ ( 17.24 ± 2.60) vs ( 10.78 ±3.41 ), t = 10. 329, P 〈 0.011. Patients' HbAlc was (9.67±1.36) % before training, and reduced to (7.83 ±1.02) % at the 3nt month, and ( 8.70 ± 1.67) % at the 6th month, and the differences were statistically significant (t = 7. 831, 9.67, respectively; P 〈 0.01 ). The difference of HbA1 c between the 3rd month and the 6th month was also significant ( t = 8.70, P 〈 0.01 ). Conclusions Empowerment education can improve the self-management ability in diabetic patients, promote their behavior change, and favor blood glucose control.
