摘要
目的利用经会阴实时三维(四维)超声对孕晚期初产妇盆膈裂孔进行观察,与其阴道分娩第二产程对比分析,探讨二者之间的相关性。方法选取无相关盆底功能障碍的初产妇40例,孕晚期入院后行经会阴四维超声检查,获取动态容积数据,观察盆膈裂孔在不同时相形态结构,追踪其自然分娩第二产程时间。结果所有病例均能清晰进行盆底四维超声成像。对盆底四维超声成像显示所有被检者耻骨弓角度为(100.7±10.9)°,盆膈裂孔面积为(13.8±3.5)cm2、盆膈裂孔周长为(14.7±3.2)cm,缩肛期面积改变量为(22.7±10.9)%、周长改变量为(16.9±8.6)%,张力期面积改变量为(23.7±13.6)%、周长改变量为(15.6±10.2)%,第二产程时间为(1.0±0.4)h。结论经会阴四维超声能够清晰显示孕晚期初产妇盆底裂孔形态,可观察不同时相盆膈裂孔形态变化,并可计算其面积及周长的变化量,统计结果显示盆膈裂孔不同时相面积及周长的改变量与第二产程时间具有一定的相关性,缩肛期面积、周长、张力期面积、周长的改变量与第二产程时间均呈负相关。
Objective To observe pelvic diaphragm hiatus of primiparas in late pregnancy with perineal real-time three-dimensional (four-dimensional) ultrasonography, contrasting with second labor stage of vaginal delivery and then to explore correlation between them. Methods 40 primiparas without related pelvic floor dysfunction were chosen and perineal dynamic four-dimensional ultrasonogmphy was performed to acquire dynamic volume data, get morphologic characters of pelvic diaphragm hiatus in different phases and track the second labor stage time in vaginal delivery. Results All cases are able to be imaged clearly by pelvic floor four-dimensional ultrasound. The images show that suprapubic angles of all examinates were (100.7±10.9)°, areas and perimeters of pelvic diaphragm hiatus were (13.8±3.5) cm2 and (14.7±3.2) cm respectively; variations of areas and perimeters in anal contraction period are (22.7±10.9) % and (16.9±8.6) % respectively; variations of areas and perimeters in tension period are (23.7±13.6)% and (15.6±10.2)% respectively; the time of the second labor stage is (1.0±0.4) h. Conclusion Perineal four-dimensional ultrasonography can clearly display the morphology of pelvic diaphragm hiatus of late pregnant primiparas, observe their changes in diverse phases and calculate variables on their areas and perimeters. Statistical results reveal that variables of areas and perimeters of pelvic diaphragm hiatus among different phases have certain correlation with the time of the second labor stage, that is, in both anal conlraction period and tension period, variables of areas and perimeters are negatively correlated with the second labor stage time.
