摘要
目的探讨细胞周期蛋白依赖性激酶抑制蛋白27（p27^KiP1）在胃癌中的表达及其与基因启动子甲基化的关系。方法采用免疫组织化学链霉菌抗生物素蛋白．过氧化物酶（SP）法检测97例胃癌标本p27^KiP1蛋白的表达，并选取25例正常胃黏膜组织作对照，同时应用甲基化特异性聚合酶链反应（MSP）方法对启动子甲基化进行检测。结果正常胃黏膜组织中p27^KiP1蛋白表达率明显高于胃癌组（X2=31．02，P〈0．01）。在高／中分化组、黏膜层／黏膜下层组和无淋巴结转移的病例中，p27^KiP1蛋白表达率分别为66．67％（30／45）、92．86％（13／14）、79．07％（34／43），但随着胃癌分化程度的降低，侵及肌层／浆膜层或发生淋巴结转移，p27^KiP1蛋白表达率明显降低，分别为30．77％（16／52）、39．76％（33／83）、22．22％（12／54）（，值分别为12．47、13．55、31．03，P〈0．01）。46例p27^KiP1蛋白高表达病例中仅有5例出现启动子甲基化，而在51例p27^KiP1蛋白低表达病例中启动子甲基化高达21例（X2=11．32，P〈0．01）。结论p27^KiP1与胃癌发生发展密切相关。p27^KiP1蛋白表达降低常提示胃癌的低分化、高侵袭转移能力。p27^KiP1基因启动子甲基化可以引起p27^KiP1蛋白表达下调或缺失．与胃癌的发牛发展密切相关．
Objective To explore the relationship between the expression of cyclin dependent kinase inhibitor protein 27 （ p27^KiP1 ） and promoter Methylation of primary gastric carcinoma. Methods Expression of p27^KiP1 was detected by using streptavid-in-peroxidase（SP） immunohistochemical staining, and promoter methylation was assayed by using methylation-specific polymerase chain reaction （MSP） in 97 cases of carcinoma, and 25 cases of normal gastric tissues. Results The expression of p27^KiP1 in normal gastric cases was higher than in gastric carcinoma tissues with the difference being significant （X2 =31.02, P 〈 0.01 ）. The expression of p27^KiP1 was related with tumor differentiation, depth of infiltration and lymph node metastasis （X2 = 12.47, 13.55, 31.03 ;P 〈0. 01）. There was a inverse association between the ex- pression of p27^KiP1and promoter methylation （X2 = 11.32, P 〈 0. 01 ）. Conclusion The expression of p27^KiP11 was associated with the occurrence and development of gastric carcinoma. The down-regulation of p27^KiP1 always suggested poor differentiation, high invasive and metastatic capability of this cancer. It may be helpful to evaluate malignant degree, invasive and metastatic potentials of gastric carcinoma. The promoter methylation of p27^KiP1 might lead to the down- or loss-exprssion of p27^KiP1 and was associated with the occurrence and development of gastric carcinoma.
