摘要
目的探讨血流变水平与急性脑梗死患者神经功能缺损恢复程度的关系。方法选取2011年1月~2012年1月我院收治的急性脑梗死患者100例,测定其血流变水平,根据血流变水平分为高血流变组54例与正常血流变组46例;从入院开始追踪观察6个月,采用中国卒中量表(CSS)和日常生活能力(ADL)评定疗效。结果高血流变组全血黏度值、血浆黏度值、血细胞比容、全血高切还原黏度、全血低切还原黏度、红细胞聚集指数与正常血流变组比较,差异均有高度统计学意义(均P<0.01);但红细胞沉降率两组比较,差异无统计学意义(P>0.05)。两组患者治疗后CSS及ADL评分均优于治疗前,差异均有统计学意义(均P<0.05),且正常血流变组两项评分均优于高血流变组,差异均有统计学意义(均P<0.05)。结论血流变水平越高,脑梗死患者神经功能缺损恢复越差,控制血流变水平可提高患者生命质量。
Objective To investigate the relationship between the level of blood flow changes and recovery of neurological deficit degree in patients with acute cerebral infarction.Methods 100 cases of patients with acute cerebral infarction in our hospital from January 2011 to January 2012 were selected,the level of blood flow changes was measured,the patients were divided into high blood flow change group(n = 54) and normal blood flow change group(n = 46) according to the level of blood flow changes;all the patients were followed-up and observed for 6 months from admission,the efficacy was evaluated by Chinese stroke scale(CSS) and activities of daily living(ADL).Results The differences of whole blood viscosity,plasma viscosity,hematocrit,high shear reduced whole blood viscosity,whole blood reduced viscosity,erythrocyte aggregation index between high blood flow change group and normal blood flow change group were all statistically significant(P 0.01);but the differences of erythrocyte sedimentation rate between the two groups was not significantly different(P 0.05).The CSS and ADL scores in patients of high blood flow change group and normal blood flow change group after treatment were all better than those before the treatment,the differences were all statistically significant(all P 0.05),CSS and ADL scores of normal blood flow change group were better than those of high blood flow change group,the differences were all statistically significant(all P 0.05).Conclusion The higher the level of blood flow changes is,the worse the recovery of the neurological deficit degree will be,controlling the level of blood flow changes can improve the quality of patients' quality of life.
