摘要
目的探讨Salter骨盆截骨术联合股骨近段旋转截骨术对小儿发育性髋关节脱位的治疗效果。方法选取本院应用Salter骨盆截骨术联合股骨近段旋转截骨术治疗小儿发育性髋关节脱位病例共26例(29髋)。按照年龄不同分为A组和B组,所有患者术前均常规进行内收肌切断和股骨髁上牵引术。术后应用改良的McKay评定标准和Severin评定标准分别对髋关节功能和影象学资料进行评估。结果 A组治疗效果略优于B组,但差异无统计学意义。所有患者均未出现皮肤及深部组织感染、坐骨神经损伤以及髂骨植骨块骨折和移位等。结论对于年龄小于6岁的发育性髋关节脱位的患者,一期行骨牵引和内收肌切断术,二期行Salter骨盆截骨术及股骨近段旋转截骨术是一种良好的治疗方案。
Objective To explore treatment effect of salter pelvis osteotomy combined femoral nearly section rotating osteotomy for pediatric developmental dislocation of the hip. Methods Twenty-six cases （29 hips） of patients with salter pelvis osteotomy combined femoral nearly section rotating osteotomy with the treatment of infantile developmen- tal dislocation of the hip were selected. The patients were divided into two groups according to the age, all the patients were given conventional for adductor cut off and femoral condyle on traction in postoperatively. The modified Mc Kay evaluation criteria and Severin evaluation standard on hip function and imaging material were assessed. Results The therapeutic effect in group A was slightly higher than that of group B, but the difference was not statistically signifi- cant. All patients were found with no skin and deep tissue infection, sciatic nerve injury and iliac crest host bone frac- tures and shift, etc. Conclusion For developmental dislocation of hip patients with age less than 6 years old, one phase bone traction and adductor amputation, two phase salter pelvis osteotomy and femoral nearly section rotating os- teotomy is a kind of good treatment.
