摘要
目的探讨中孕期超声测量胎儿颈部皮肤皱褶(nuehalfold,NF)厚度的临床意义。方法对1984例中孕期(孕18~24周)产前超声筛查孕妇测量NF厚度,NF厚度≥6.0mm为NF增厚组,NF厚度〈6.0mm为对照组,观察NF增厚与胎儿染色体和胎儿结构异常的关系。结果1984例胎儿共发现NF增厚41例(NF增厚组),余1943例NF正常(对照组)。NF增厚组41例胎儿中26例检出胎儿结构异常(63.41%),9例有胎儿染色体异常(21.95%)。对照组1943例检出胎儿结构异常20例(1.03%),306例有产前诊断指征者羊膜腔或脐带穿刺检查发现胎儿染色体异常4例(1.31%,4/306)。NF增厚组胎儿结构异常率明显高于对照组(P〈0.01),胎儿染色体异常检出率明显高于对照组中有产前诊断指征者(P〈0.05)。结论胎儿NF增厚是中孕期超声筛查胎儿染色体异常的重要超声指标,胎儿NF增厚容易合并其他结构异常。
Objective To explore the clinical significance of fetal nuchal fold(NF) thickness in the second trimes- ter of pregnancy measured by ultrasound. Methods NF thickness was measured for prenatal ultrasound screening in 1 984 pregnant women in the second trimester of pregnancy (pregnancy at 18 -24 weeks). NF thickness≥6 mm was as- signed to NF thickened group, NF thickness 〈 6 mm was assigned to control group. The relationships between NF thick- ening and fetal chromosomal abnormalities as well as structural fetal anomalies were observed. Results Thickened NF was found in 41 cases among 1 984 fetuses (NF thickened group) , while the remaining 1 943 cases had normal NF thick- ness( control group). Structural fetal anomalies were detected in 26 cases in NF thickened group (63.41%), and fetal chromosomal abnormalities were found in 9 cases (21.95%). 20 cases of structural fetal anomalies were detected in con- trol group of 1 943 fetuses ( 1.03% ) , 306 eases were punctured through amniotie sac puncture or umbilical cord punc- ture conforming to prenatal diagnosis indications, and 4 cases were revealed of fetus chromosomal abnormalities (1.31% , 4/306). The rate of structural fetal anomalies in NF thickened group was significantly higher than that of control group ( P 〈 0. 01 ). The detection rate of fetal chromosomal abnormalities in NF thickened group was obviously higher than those who had prenatal diagnosis indications in control group ( P 〈 0.05 ). Conclusion Fetal NF thickening is an important ultrasound indicator in the second trimester of pregnancy screening for chromosomal abnormalities. Fetal NF thickening is often accompanied by other structural fetal anomalies.
