518例骨盆骨折患者生存概率基本特征分析 被引量:3

摘要 目的描述宁夏银川地区骨盆骨折患者生存概率的基本特征,探讨影响患者生存概率的可能因素。方法回顾收集和分析宁夏银川地区518例骨盆骨折患者病历资料,计算改良创伤评分(RTS)、创伤严重度评分(ISS)、最后用创伤与损伤严重度评分(TRISS)法计算得到生存概率(PS),采用描述性方法分析不同特征患者生存概率的差异。结果本组骨折男性366例,女性152例,男女比例为2.4∶1。(1)男性RTS值为(3.86±0.4112),女性为(3.86±0.4214);男女性患者ISS值分别为(22.67±10.4961)、(21.14±9.5876);男女性PS值分别为(0.61±0.2289)、(0.62±0.2214);性别之间RTS值、ISS值、PS值差异均无统计学意义。(2)以年龄划分,≥55岁组和<55岁组之间RTS和PS差异有统计学意义。(3)通过影响PS因素的多元逐步回归分析发现,影响PS的因素包括年龄、舒张压、RTS、ISS、综合评分方法(CRAMS)。结论 RTS、ISS是评判宁夏银川地区骨盆骨折严重程度的有效指标之一,依据评分可以对伤者病情做出及时有效的判断,从而为提高救治水平提供一定的帮助。 Objective To explore the features suivival of probability in cases with pelvic fractures in Ningxia.Methods Clinical information in 518 cases with pelvic fracture were collected and analyzed.The RTS,ISS and PS value were calculated by TRISS method.Results 366 male cases and 152 female cased were included in this research and the sex ratio is 2.4∶1.(1) The RTS valuewas(3.86±0.4112) in males and was(3.86±0.4214)in females(P〈0.05).The ISS values was(22.67±10.4961) in males and(21.14±9.5876) in females(P〈0.05).The Ps value was(0.61±0.2289) and(0.62±0.2214) in males and females respectively(P〈0.05).(2)RTS showed significant difference between the ≥55 age group and the55 age group.(3)The age,diastolic blood pressure,CRAMS,RTS,ISS were affecting factors of Ps by regression analysis.The regression equation was Y = 0.225-0.397 × age of +0.0003 × diastolic blood pressure +0.002 × CRAMS +0.193 × RTS-0.015 × ISS.Conclusion RTS,ISS are judging factors for severity of pelvic fractures in Ningxia.The affecting factors of survival probability are age,diastolic blood pressure,CRAMS,RTS,ISS.
