有创机械通气治疗重症急性心源性肺水肿的临床观察 被引量:7

摘要 目的观察探讨有创机械通气治疗重症急性心源性肺水肿(ACPE)的疗效和临床应用价值。方法回顾性分析33例重症ACPE患者在常规治疗基础上应用有创机械通气,观察通气前后临床症状变化及相关指标变化。结果其中29例患者抢救成功,有效率为87.9%(29/33),经有创机械通气治疗后心率、呼吸频率、平均动脉压、血气pH值、经皮动脉血氧饱和度(SaO2)、动脉氧分压(Pa02)上升,动脉二氧化碳分压(PaC02)下等明显改善,治疗前及治疗后2h两组指标相比有统计学意义(P<0.05)。结论对于严重的重症急性心源性肺水肿患者,在常规治疗的同时积极行有创机械通气是成功抢救的关键。 Objective To observe the efficacy of the invasive mechanical ventilation in the treatment of severe ACPE. Methods Retrospective analysis of 33 patients with severe ACPE applied invasive mechanical ventilation on the basis of convcn- lional treatment. Results The 29 patients rescue success, effective rate was 87.9% (29/33) , and by a mechanical ventilation therapy heart rate(HR) and respiratory rate(R), arterial blood gas pH, arterial partial pressure of oxygen( Pa02 ), arterial carbon dioxide partial pressure( PaC02 ) nmrkedly improved, a statistical differences is showed ( P 〈 0.05). With tile treatment of invasive mechanical ventilation 29 patients go to better,The total effective rate 87. 9% (29/33). Conclusion lnvasive mechanical ventila- tion is an effective supportive measure to rescue severe ACPE that can rapidly improvepatient symptoms.
