摘要
目的观察骨化三醇胶丸冲击疗法联合高通量血液透析(HFHD)治疗维持性血液透析患者继发性甲状旁腺功能亢进(SHPT)的疗效。方法对15例SHPT患者在常规血液透析(HD)基础上采用骨化三醇胶丸冲击疗法联合HFHD,疗程3个月,观察治疗、清甲状旁腺素(PTH)、血钙(Ca)、血磷(P)变化,以及皮肤瘙痒和关节、肌肉痠痛改善情况。结果 PTH治疗前(952.2±170.9)pg/mL,治疗后(287.6±89.3)pg/mL;血Ca治疗前(1.7±0.4)mmol/L,治疗后(2.4±0.3)mmol/L;血P治疗前(2.5±0.2)mmol/L,治疗后(1.4±0.5)mmol/L,治疗前后比较差异均有统计学意义(P<0.05);皮肤瘙痒改善有效率93.3%,关节、肌肉痠痛改善有效率66.7%。结论骨化三醇胶丸冲击疗法联合HFHD可明显降低维持性血液透析SHPT患者血清PTH、血P水平,升高血Ca水平,改善皮肤瘙痒及关节、肌肉痠痛症状,治疗SHPT疗效确切。
Objective To investigate the effect of calcitriol soft capsule pulse therapy combined with high-flux hemodialysis treatment in secondary hyperparathyroidism（SHPT） of maintenance hemodialysis patients.Methods 15 cases of SHPT were treated by calcitriol soft capsule pulse therapy combined with high-flux hemodialysis treatment on the basis of the conventional hemodialysis（HD） for three months of treatment.The changes of parathyroid hormone（PTH）,calcium（Ca）,phosphorus（P） in serum before and after treatment,and improvement of itchy skin,joints and muscles pain were observed.Results PTH in serum was（952.2±170.9） pg/mL before treatment and（287.6±89.3） pg/mL after treatment.Ca in serum was（1.7±0.4） mmol/L before treatment and（2.4±0.3） mmol/L after treatment.P in serum was（2.5±0.2） mmol/L before treatment and（1.4±0.5） mmol/L after treatment.There were significant differences between before and after treatment（P〈0.05）.Improvement efficiency of itchy skin and joints and muscles pain was 93.3% and 66.7%.Conclusion Calcitriol soft capsule pulse therapy combined with high-flux hemodialysis treatment can significantly reduce serum PTH,blood P levels in maintenance hemodialysis SHPT patients,elevate blood Ca levels,improve skin itching and pain symptoms of joint and muscle,and has definite effect in the treatment of SHPT.
