摘要
目的 :探讨侵及眼眶的晚期上颌癌 (T3 ～T4 )根治手术时 ,眼眶内容物的保存适应症。方法 :对 1986年 1月～ 1996年 1月收治的 32例晚期上颌癌患者的临床特点、治疗方法及追踪结果进行回顾性总结。根据患者术前高分辨力CT和 /或MRI扫描检查结果及手术中探查情况分成三组。比较了三组病例根治手术时保存眼眶内容物的条件和术后生存率。结果 :三组不同临床表现的有限病例 ,保存眶内容的根治术后作了追踪检查。复发率虽有不同但所有复发部位均不在眼眶附近或眼眶内。结论 :晚期上颌癌 (T3 ～T4 )侵及眼眶后 ,大部分病例行根治手术而保存眼眶内容物仍可取得较佳疗效而不违反肿瘤根治原则。部分病例 ,经术前放疗后 ,施行根治手术时保存眼眶内容仍是可能的。
Objective:[WT5”BZ] To evaluate the management of the orbital contents during radical surgery for advanced maxillary cancer (T 3～T 4) with orbital involvement. [WT5”FZ]Methods:[WT5”BZ] 32 cases of advanced maxillary cancer (T 3～T 4) with orbital involvement were treated form Jan. 1986 to Jan. 1996 and their clinical characteristics, treatment and prognosis reviewed retrospectively.According to the results examined by high-resolution computed tomographic and/or magneyic resonance imaging scans and the conditions examined in the operation,the whole cases were divided into three groups.The conditions for preservation of the orbital contents during radical surgery and ultinate survival of three groups were compared [WT5”FZ]Resuits:[WT5”BZ]The limited cases of three groups with diffrent clinical characteristic after radical surgery were followed up respectivelv.Although the recurrence rates wrer different postoperatively,all the cases with tumor recurrence had not recurred in or around the orbit,[WT5”FZ]Conclusion:[WT5”BZ]A large part of cases with the advanced maxillarv cancer(T 3～T 4)invading the long orbit,the orbital contents can be preserved after radical surgery and still get a good results without compromising oncologic safety,Some selected case with tumor invading into the orbit after prloperative radiotherapy,their orbital contents still can be preserved possibly during radical surgery.
