改良“三步雾化吸入”技术治疗毛细支气管炎的临床疗效观察 被引量:5

摘要 目的观察改良"三步雾化吸入"技术治疗毛细支气管炎的临床疗效。方法 160例住院的毛细支气管炎患儿随机分成两组,对照组(70例)采用传统的雾化吸入技术,试验组(90例)采用改良"三步雾化吸入"技术:第一步给予3%高渗盐溶液雾化吸入,诱导排痰;第二步叩背、吸痰,加强雾化前后的呼吸道管理,第三步予以普米克令舒和博利康尼雾化液两者联合雾化吸入。两组均通过氧气驱动雾化,观察治疗前后喘息症状和肺部哮鸣音消失时间以及住院时间,进行比较。结果试验组在缩短喘息、肺部哮鸣音以及住院时间上均较对照组差异显著,P<0.05。未发现与用药相关的副作用。结论采用改良"三步雾化吸入"技术能有效清除呼吸道分泌物,提高雾化药物吸入效果。在控制喘憋、修复气道及缩短肺部哮鸣音时间上更有效且安全。 Objective To observe the clinical effect of the modified three-step aerosol inhalation technique in the treatment of bronchiolitis. Methods 160 patients with bronchiolitis were randomly divided into two groups. The control group (n = 70) were treated with the traditional aerosol inhalation technique, and the test group (n = 90) were treated with the modified three-step aerosol inhalation technique. The first step was to give 3% hypertonic saline inhalation to induce sputum. The second step was to percuss back and suction so as to strengthen the management of respiratory tract. The third step was to inhale the atomized liquid combination of Pulmicort and Ter- butaline. The two groups were both driven by the oxygen atomization. The wheezing symptoms, pulmonary wheezing vanishing time and duration of hospital stay were observed before and after the treatment between the two groups. Results The improvement was more pro- nounced in the test group than in the control group with statistical significance ( P 〈 0.05 ), and there was no side effect related with drug. Conclusion The modified three-step aerosol inhalation technology can effectively clear airway secretions and improve the effect of atomi- zing inhalation. It is more effective and safer to control asthma, repair airway and shorten the time of pulmonary wheezing.
