摘要
目的探讨腹膜前疝修补装置在腹股沟疝修补术中的疗效及技术要点。方法总结云南省第一人民医院2010年11月至2012年7月在应用各型腹膜前疝修补装置行前入路腹膜前修补,治疗腹股沟疝282例的临床资料。结果本组282例患者,手术时间40～90min,平均50min,术后有尿潴留4例,阴囊积液3例,阴囊血肿1例。切口疼痛轻微,无明显异物感,无切口感染等并发症,术后3～5d出院。随访1～19个月未见复发。结论腹膜前间隙补片修补耻骨肌孔是对腹股沟区的全面修补,具有手术彻底,术后恢复快,并发症少,复发率低的优点,从根本上解决腹股沟疝患者各种疝并发及容易复发的病理生理特点,是目前腹股沟疝治疗中转理想的术式。
Objective To investigate the effectiveness and technical keys of preperitoneal hernia repair device in inguinal hernia repair. Methods A total of 282 cases who had inguinal hernia and were ap- plied with peritoneal hernia repair device before peritoneum were collected from November 2010 to July 2012. Results Of all 282 patients,the operation time lasted 40 to 90 minutes with a mean of 50 minutes. Four cases of postoperative urinary retention,3 cases of scrotal effusion, 1 case of scrotal hematoma were ob- served. The incision was found to have minor pains without any obvious sensation of foreign body and compli- cations of incision infection. The patients can be discharged 3 to 5 days after operation and no recurrence was observed after 1 to 19 months follow ups. Conclusions Comprehensive repair of preperitoneal space patch for the area of pubic muscle hole had advantages of rapid recovery, less complications, low recurrence rate. The technical method was based on the pathophysiologic characteristics of inguinal hernia and complications, which was ideal for treatment of inguinal hernias.
