摘要
[目的]介绍腹膜透析病人管理的个案管理模式并探讨对其应用效果。[方法]采用案例研究,以上海市某三级甲等综合性医院肾内科为现场,对腹膜透析病人、医生及专科护士进行为期2周的非参与式观察结合半结构式访谈。[结果]个案管理模式获得医生、护士、病人及家属的认可,运用个案管理模式对病人进行全程全面管理,提高了病人治疗和护理的依从性和生活质量,提升了团队合作默契和凝聚力,提高了工作效率。[结论]个案管理护理模式是腹膜透析病人管理的有效工作模式之一。
Objective.To introduce the case management model in manage- ment of peritoneal dialysis patients and probe into its applied effect. Methods：By using case studies and taking the department of Nephrology in a three grade A general hospital in shanghai city as the scene, the peri- toneal dialysis patients,doctors and specialist nurses received a period of two weeks of non - participant observation combined with semi - struc- tured interviews. Results：Case management model got the recognition of doctors, nurses, patients and their families, and of the patient for the en- tire using case management model for overall management of patients improved the patients＇compliance to treatment and care and their quality of life, enhance the tacit understanding and cohesion of teamwork and im- prove the work efficiency. Conclusion： Case management nursing model is one of the effective work patterns in peritoneal dialysis patient management.
