摘要
目的讨论脑积水经脑室-腹腔分流术后分流系统梗阻的原因及其防治方法。方法回顾性分析2006年1月至2012年1月53例脑积水脑室-腹腔分流术治疗分流系统梗阻患者的临床资料。结果脑室端梗阻27例,分流泵和储液囊梗阻7例,颈、胸、腹段引流管梗阻4例,腹腔端梗阻15例。53例患者经调整或更换分流系统后脑室系统较术前缩小,48例恢复良好,1例因严重感染死亡,1例术后即转回当地医院康复,3例再发梗阻调整后好转。结论规范的手术操作、良好的无菌观念、及时的健康教育是预防分流系统梗阻的关键。
Objective To discuss the cause, prevention and treatment of the shunt system obstruction (SSO) after the ventriculo- peritoneal shunt (VPS) for the hydrocephalus. Method The clinical data of 53 patients with hydrocephalus, in whom SSO occurred after VPS performed from January, 2006 to January, 2012, were analyzed retrospectively. Results SSO occurred at the ventricular tips of the shunt system in 27 patients, in the diverters and the bags for storing liquid in 7, in the cervical, thoracic and abdominal segments in 14, at the peritoneal tips in 14. The cerebral ventricle shrank after the shunt system was adjusted or replaced in 53 patients, of whom, 48 were recovered well, 1 died from severe systemic infection, 1 was translated to the local hospital for the rehabilitation, and 3 who had recurrent obstruction of the shunt system after the adjustment were recovered well after the readjustment of the shunt system. Conclusion The standardized operative procedure, good aseptic concept and timely health education are the key to preventing the obstruction of the shunt system in the patients with hydrocephalus undergoing VPS.
