摘要
目的了解2007-2011年医院革兰阴性菌的临床分布及耐药性,以指导临床合理使用抗菌药物,降低细菌耐药性的产生。方法采用VITEK-60全自动微生物分析仪和配套的革兰阴性鉴定卡、革兰阴性药敏卡-143、革兰阴性药敏卡-448,对5745株革兰阴性杆菌进行分离鉴定和药敏试验,药敏结果使用WHONET5.5软件进行统计分析。结果革兰阴性菌前8位菌株依次为大肠埃希菌、肺炎克雷伯菌、副流感嗜血菌、流感嗜血菌、鲍氏不动杆菌、铜绿假单胞菌、奇异变形菌、阴沟肠杆菌,分别占38.8%、18.4%、9.9%、9.5%、8.3%、6.0%、4.8%、4.3%;大肠埃希菌中产超广谱β-内酰胺酶菌株占50.5%、肺炎克雷伯菌中占48.0%;阴沟肠杆菌、肺炎克雷伯菌和奇异变形菌对亚胺培南敏感。结论对耐亚胺培南鲍氏不动杆菌、铜绿假单胞菌和肠杆菌科细菌,产ESBLs的肺炎克雷伯菌和大肠埃希菌,应加强隔离预防,根据药效试验结果合理用药,控制耐药菌的产生。
OBJECTIVE To study the clinical distribution and drug resistance of the gram-negative bacteria from 2007 to 2011 so as to guide the reasonable clinical use of antibiotics and reduce the emergence of bacterial resistance. METHODS By means of VITEK 60 automated microbial analyzer and supporting a GNI, GNS-143, GNS-448, the isolation and identification as well as the drug susceptibility testing of 5745 strains of gram-negative bacilli were performed, and the statistical analysis of the results of the drug susceptibility testing was carried out by using WHONET5. 5 software. RESULTS Escherichia coli （38. 8%）, Klebsiella pneumoniae （18. 4%）, Haemophilus parain fluenzae （ 9. 9 % ） , Haemophilus in fluenzae （9. 5 % ） , Acinetobacter baurnannii （8. 3 % ） , Pseudomonas aeruginosa （6.0%）, Proteus mirabilis （4.8%）, and Enterobacter cloacae （4.3%） were the top 8 species of gram-negative bacteria. The extended spectrum β-lactamase （ESBLs）-produeing E. coli strains accounted for 50.5%, and the ESBLs-producing K. pneumoniae strains accounted for 48.0%. E. cloacae, K. pneumoniae and P. mirabilis were sensitive to imipenem. CONCLUSION It is necessary to strengthen the isolation and prevention of the imipenem-resistant A. baumannii, P. aeruginosa and Enterobacteriaceae as well as the ESBLs-producing K. pneumoniae and E. coli and to reasonably use antibiotics on the basis of the drug susceptibility testing result so as to control the drug resistant strains.
