脑卒中患者急性期糖代谢异常及肺部感染的临床分析 被引量:2

Clinical analysis of acute phase of abnormal glucose metabolism complicated with pulmonary infections in stroke patients

摘要 目的探讨脑卒中患者急性期糖代谢异常及并发肺部感染的临床分析,为早期防治提供指导。方法采用前瞻性研究法收集2010年4月-2011年12月医院神经内科收治的脑卒中患者310例,分析急性期糖代谢异常在年龄、卒中类型、肺部感染等分布情况。结果脑卒中患者急性期糖尿病及糖耐量受损发病率高;年龄≥60岁、体质指数增高、具有高血压、高血脂病史、NIHSS评分≥7分者糖代谢异常发生率分别高于年龄<60岁、缺血性卒中、NIHSS评分<7分、无高脂血症、体质指数<30kg/m2的脑卒中患者,差异有统计学意义(P<0.05)。结论脑卒中患者急性期糖代谢异常的发生率较高,年龄、体质指数和高脂血症是糖代谢异常的重要因素。 OBJECTIVE To study the acute phase of abnormal glucose metabolism in the stroke patients complicated with pulmonary infections so as to guide the early prevention. METHODS By means of the prospective survey, a total of 310 stroke patients who enrolled the neurology department of Yiwu hospital from Apr to Dec 2011 were selected as the research objects, and the distribution of the patients with the acute phase of abnormal glucose metabolism in the age, type of stroke, and the pulmonary infections was analyzed. RESULTS The incidence rate of the acute phase of diabetes or impaired glucose tolerance was high in the stroke patients; the incidence rate of the abnormal glucose metabolism was higher in the patients with more than 60 years of age, the patients with elevated body mass index, the patients with hypertension history, the patients with hyperlipidemia history,or the patients with the NIHSS score more than 7 points than in the patients with less than 60 years of age, the patients with ischemic stroke, the patients without hyperlipidemia history, the patients with the NIHSS score less than 7 points, or the patients with the body mass index less than 30 kg/m2, the difference was statistically significant (P〈0.05). CONCLUSION The incidence rate of the acute phase of abnormal glucose metabolism is high in the stoke patients, and the age, body mass index and hyperlipidemia are the main factors associated with the abnormal glucose metabolism.
