摘要
目的观察乳突半开放并填充术治疗胆脂瘤中耳炎的疗效,探讨无鼓室成形条件的胆脂瘤中耳炎的外科治疗方法。方法 2009年11月～2011年10月对58例(58耳)无鼓室成形术条件的胆脂瘤中耳炎患者行乳突半开放、上鼓室切开、骨粉填充术腔手术,术后随访6～9个月,观察其近期效果。结果 58例(58耳)患者手术时间均在1小时内,术后1个月左右术腔完全上皮化,外耳道形态完全恢复,手术前后听力无明显改变。除1耳术后出现感染治疗后痊愈,5例出现蝉鸣样耳鸣,治疗2月效果不佳外,无其他并发症。结论乳突半开放并填充术简化了手术操作复杂程度,缩短了手术时间和术后愈合时间,对无鼓室成形术条件的中耳胆脂瘤患者是一种可选择的手术方式。
Objective To discuss the surgical technique in the treatment of cholesteatomatous otitis media without tympanoplasty. Methods This article describes the semi--open mastoidectomy with filling in the treatment of cholesteatomatous otitis media in our hospital from November 2009 to October 2011, preliminary summary the short--term effect of surgical treatment of cholesteatoma otitis media. Results All cases were followed up for six to nine months. 58 cases were healed by 2~3 dressing postoperatively, completely epithelialization about one month postoperatively, external auditory canal morphology was fully restored, healed well. Conclusion Semi--open mas toidectomy with filling in thetreatment of cholesteatomatous otitis media improved to simplify the complexity of the classic mastoidectomy , shortened the operation time significantly, shortened postoperative healing time. However, long--term effect remains to be seen.
