摘要
目的通过在延安大学附属医院开展妊娠期糖尿病的筛查,进行针对性治疗,减少孕、产期并发症,提高孕产妇及新生儿的健康水平。方法从2010.1～2012.1对我院门诊产前检查24～28周自愿接受糖筛的孕妇1470人按有无糖尿病高危因素进行分组,将无糖尿病高危因素的分为A组共930人,将有糖尿病高危因素孕妇分为B组共540人,糖筛的方法:空腹血糖(FPG)+75 gOGTT,必要时查HbA1c。结果 A组,失访,460人,通过常规产前检查无异常,再进行空腹血糖(FPG)+75 gOGTT筛查,筛出妊娠期糖尿病3人,筛出率为0.3%,失访率高达49.4%,B组,失访4人,有糖尿病高危因素,或通过常规产前检查尿糖阳性,检出妊娠合并糖尿病112人,筛出率为20.7%,失访率0.7%,患者依从性好,以上均有显著性差异,P<0.05。结论延安地区妊娠期糖尿病筛查,对常规产前检查尿糖阳性及有糖尿病高危因素的患者行空腹血糖(FPG)+75 gOGTT,必要时查HbA1c方法筛查妊娠期糖尿病,简单,经济,检出率高,患者容易接受,降低孕产妇死亡率和新生儿患病率,从而提高妇女儿童健康水平,值得推广。
Objective To reduce complications of pregnancy and childbirth and tto improve maternal and newborn health by screening for gestational diabetes mellitus. Method From 2010 -01 -2012 -01, 1470 of 24 ~ 28 week pregnant women access to voluntary screening for gestational diabetes mellitus were grouped according to with or without risk factors of diabetes ; no risk factors are divided into group A, a total of 930, rick factors into group B, a total of 540 people. Screening include gestational diabetes millitus FPG ＋ 75g OG33＂. In necessary , cheek the glyyo- sylated hemoglobin. Results the group of A,460 persons lost to follow -up, by routine antenatal cheeks no excep- tions for FPG ＋ 75gOGTT, Detected in patients with gestational diabetes mellitus 3 people, detection rate of 0.3 per cent,loss to follow- up of 49.4 per cent. The group of B ,4 persons lost to follow -up, people who have diabetes risk factors , through positive for urine routine inspection identified people , 112 people confirmed patients , detec- tion rate of 20.7 per cent , loss to follow -up of 0.7 per cent. The compliance of patients with good , compared to both have sighificant differences, P 〈 0.05. Conclusion screening for GDM in Yan＇ an area , regular antenatal urine positive and risk factors in patients with FPG ＋75g OGTT in necessary check the glycosylated hemoglobin methods , simple, economic, high detection rate, patients with affordable, reducing maternal mortality and neonatal morbidi- ty, so as to improve the health of women and children, so it＇s worth promoting.
