摘要
目的探讨胃镜在上消化道出血患者的病因与诊治中的应用。方法对我院2011年1月至2012年1月期间82例上消化道出血患者的临床资料进行回顾性分析,总结胃镜在其病因诊断以及治疗中的应用;分析不同时间行胃镜诊断的检出率差异以及不同治疗方法的有效率。结果上消化道出血主要病变有溃疡50例(其中十二指肠溃疡26例,胃溃疡16例,胃肠复合溃疡8例),其次为急性胃粘膜病变11例,胃癌9例;经胃镜检查后明确病因的有78例,检出率为95.1%,其中48小时检出率100.0%,48小时后检出率为88.5%,检出率差异有统计学意义(P<0.05);74例患者均成功止血,止血率为94.5%。其中喷药止血患者59例,成功止血56例,成功率94.9%;注射止血患者19例,成功率94.7%;两种方法成功率比较,差异无统计学意义(P>0.05)。结论胃镜是诊断和治疗上消化道出血有效的临床方案,值得临床推广使用。
Objective To explore the application ofgastroscope in the diagnosis and treatment of upper gastrointestinal hemorrhage. Methods The clinical data of 82 cases of upper gastrointestinal hemorrhage in our hospital from January 2011 to January 2012 were analyzed retrospectively. The application of gastroscope in etiologic diagnosis and treatment were summarized. The detection rates at different time and the treatment effects were analyzed. Results The main causes of upper gastrointestinal hemorrhage were ulceration （including 26 cases of duodenal ulcer, 16 cases of gastric ulcer, and 8 cases of gastrointestinal composite ulcer）, followed by 11 cases of acute gastric mucosal lesions, and 9 cases of gastric cancer. The causes of the disease were diagnozed in 78 cases （95.1%）, and the detection rate was 100% within 48 hours, and 88.5% after 48 hours （P 〈0.05）. 74 patients successfully stopped bleeding, with 59 cases of drug spraying （success rate of 94.9%）, and 19 cases of drug injection （success rate of 94.5）; the difference of success rate between two methods was not statistically significant （P〉0.05）. Conclusions Gastroseope is an effective clinical program for the diagnosis and treatment of upper gastrointestinal hemorrhage, and it＇s worthy of clinical application.
