摘要 目的应用计划行为理论(TPB)预测注意缺陷多动障碍(ADHD)儿童服药依从性行为的影响因素,探讨可能的干预策略。方法基于TPB设计ADHD儿童服药依从性定量问卷,对北京市某三甲医院181例该类儿童的家长进行问卷调查,其中初诊80例、复诊101例。运用线性回归模型筛选服药依从行为意向的影响因素。结果情感态度为初诊、复诊儿童家长服药依从行为意向的最大影响因素,标准化回归系数分别为0.657、0.447。儿童年龄对初诊、复诊儿童家长行为意向均产生影响,但作用方向不同,儿童年龄越大,初诊家长行为意向越强,复诊家长行为意向越弱。初诊儿童家长行为意向还与儿童性别呈正相关,与家长年龄、家长文化程度呈负相关,标准化回归系数分别为0.156、-0.250和-0.167。结论提高ADHD儿童服药依从性可以采取的干预策略可能有尽早干预、实施系统的健康教育、政策支持、提高社会对ADHD的认知等。 Objective To predict inflnent factors of attention deficit hyperactivity disorder (ADHD) by using theory of planned behavior (TPB) , and explore appropriate intervention strategies. Methods A questionnaire about medi- cation adherence of ADHD based on TPB was designed. 181 parents of ADHD child were collected from one of the third level hospital in Beijing, 80 first visit and 101 return visit. Linear regression model was used to screen inflnent factors of medication adherence. Results The biggest influent factor for parents who was first visit or return visit was emotional atti- tude, with the standardized regression coefficient 0. 657 and O. 447, respectively. Children' s age influenced behavioral in- tention of parents who was first visit and return visit, but had different direction in the two groups. With the children' s age increasing, behavioral intention became stronger in parents with first visit while reduced in parents return visit. Among par- ents for first visit, gender of children was positive associated with behavioral intention, while parent' s age and education level were negative correlation the standardized regression coefficient was 0. 156, - O. 250 and - 0. 167, respectively. Conclusion Intervention strategies including early intervention, systematic health education, policy support and social cognition improvement of ADHD should be carried out to enhance medication adherence of ADHD.
