小儿荨麻疹心肌酶谱的变化与临床研究 被引量:2

Clinical study on the alternation of myocardiac enzymes spectrum of urticaria in children

摘要 目的探讨小儿荨麻疹心肌酶谱及超敏C反应蛋白在小儿荨麻疹诊疗过程中的临床价值。方法检测184例荨麻疹患儿心肌酶谱及超敏C反应蛋白水平,CK、CK—MB大于正常值2倍者均做肌钙蛋白和心电图检查,同时选择60例健康体检儿童作为对照组,进行研究分析。结果荨麻疹患儿组心肌酶谱与超敏C反应蛋白均明显高于对照组,差异有统计学意义(P〈0.05)。184例荨麻疹患儿中有109例心肌酶谱阳性和108例超敏c反应蛋白阳性,阳性率分别为59.2%和58.7%,其中肌钙蛋白阳性者2例,结合心电图诊断轻度心肌炎1例。结论心肌酶谱和超敏C反应蛋白在小儿荨麻疹诊断、治疗及预后评估过程中具有重要临床价值。在小儿荨麻疹合并心肌损伤或合并心肌炎的诊断上应结合症状、肌钙蛋白及心电图综合考虑做出判断,以防误诊。 Objective To observe the children with urticaria myocardial enzymes and high sensi- tivity C reactive protein, the dissertation investigates its clinical value in the diagnosis and treatment of children with urticaria. Methods Detected 184 cases of urticaria in children with myocardial enzymes and high sensitivity C-reactive protein, CK, CK-Mb two times greater than normal, do troponin and ECG, and other 60 cases of healthy children were tested as control group to research and analyse. Re- suits Urticaria in children with group of myocardial enzymes and high sensitivity C-reactive protein was significantly higher, there was significant difference ( P 〈 0. 05 ). In 184 cases of urticaria children, 109 cases of myocardial enzymes in positive and 108 cases of high sensitivity C-reactive protein positive, the positive rates were 59.2% and 58.7% , included troponin-positive 2 cases, combined with the ECG di- agnosis of mild myocarditis cases. Conclusions The myocardial enzymes and high sensitivity C-reactive protein in children with urticaria diagnosis, treatment and prognosis of the assessment process has impor- tant clinical value. It should be combined with symptoms the diagnosis of children with urticaria merger of myocardial injury or merge myocarditis, troponin and ECG to consider making the judgment and to pre- vent misdiagnosis.
