摘要
目的:探讨疏血通治疗血透患者长期留置导管晚期功能不良的疗效。方法:纳入两个中心在2010年8月至2011年11月因长期留置导管晚期功能不良需反复尿激酶导管内溶栓治疗1个月以上的血透患者。在基础治疗不变情况下予0.9%生理盐水100mL加疏血通4mL,每次透析时动脉端输注,连用1个月。收集用药前后1个月内尿激酶导管内溶栓次数及尿激酶总用量、用药前及用药1个月后凝血功能和药物不良反应。结果:共19名患者入组。对比用药前,加用疏血通后尿激酶导管内溶栓治疗次数显著减少[(1.21±0.54)次/月vs(3.35±1.02)次/月,P=0.01],尿激酶总用量显著减少[(16.78±9.36)万U/月vs(60.62±11.53)万U/月,P<0.01]。用药前后凝血功能结果差异无统计学意义。加用疏血通后未见出血并发症。结论:疏血通可减少血透患者长期留置导管晚期功能不良治疗时的尿激酶导管内溶栓次数及尿激酶用量。
Objective To investigate tile effect of shuxuetong on late dysfunetion of tunneled tufted catheters in patients underwent hemodialysis. Methods Patients who underwent hemodialysis, with late dysfunction of tunneled euffed catheters, required repeatedly urukinase intracatheter thrombolysis therapy, in two dialysis centers from August 2010 to November 2011 were em'nlled. After enrollment, in addition to unchanged basical therapy, 100 mL of 0.9% NS with 4 mL of shuxuetong were injected through the artery port at each hemodialysis procedure for 1 month. The frequency of urokinase intracatheler thrombolysis therapy and the total dosage of urokinase in 1 month, blood coagulation test results betbre and after the usage of shuxuetong, and the adverse reaction of medicine were recorded. Results 19 patients were enrolled. After the usage of shuxuetong, the frequency of urokinase intracatheter thrombolysis therapy was significantly decreased [ ( 1.21 ± 0.54) times per month vs. (3.35 ±1.02) times per month, P = 0.01 ], the total dosage of urokinase was significantly decreased [ ( 16.78 ± 9.36) ten thousand units per month (60.62 ± 11.53) ten thousand units per month, P 〈 0.011, the blood coagulation test results were no significantly different, no bleeding c.omplieations were found. Conclusion Shuxuetong can reduce the frequency of urokinase intracatheter thrombolysis therapy and the total dosage of urokinase in hemodialysis patients with late dysfunction of tunneled cuffed catheters.
