摘要
目的探讨颅脑外伤术后颅内感染的相关危险因素及其预防控制依据,以期预防和降低颅内感染的发生。方法收集1643例颅脑外伤手术治疗患者中的137例并发颅内感染者临床资料进行回顾性分析,对可能相关因素进行单因素分析,筛选后再行logistic多因素回归分析。结果单因素分析发现颅内感染与年龄、手术时间、术后切口脑脊液漏、脑室外引流、术前GCS评分、开放性伤口是影响患者颅内感染的重要因素(P<0.05),logistic多因素回归分析发现,脑室外引流、手术时间和脑脊液漏是颅脑术后颅内感染的独立高危险因素。结论颅脑外伤术后颅内感染是多个因素综合作用的结果,而脑脊液漏、脑室外引流和手术时间是术后颅内感染的独立危险因素,及早采取措施对高危因素患者进行预防性处理,有助于降低颅脑外伤术后颅内感染的发生。
OBJECTIVE To explore the risk factors and prevention measures for the postoperative intracranial infections after surgery for craniocerebral trauma so as to prevent and reduce the incidence of the intracranial infections. METHODS The clinical data of 137 patients with postoperative intracranial infections selected from 1643 patients undergoing craniocerebral trauma surgery were retrospectively analyzed. The possible risk factors associated with ICI were firstly analyzed with single factor analysis, and then analyzed with multiple logistic regression method. RESULTS Univariate analysis showed that the age, operation duration, cerebrospinal fluid leakage,ventricle external drainage, pre-operation GCS score and open wound were the risk factors for the postoperative intracranial infections in patients after craniotomy(P〈0.05). The multivariate logistic regression analysis found that the ventricle external drainage, cerebrospinal fluid leakage and operation duration were the independent high risk factors for intracranial infections after craniotomy. CONCLUSION The intracranial infections after the craniotomy are the combined effect of multiple factors, however, the ventricle external drainage, the cerebrospinal fluid leakage and the operation duration are the independent risk factors for postoperative intracranial infections. It is conducive to reduce the incidence of intracranial infections after the surgery for craniocerebral trauma to take measures in the early stage for preventive treatment of the patients at high risk.
