摘要
目的探讨鼻结外NK/T细胞淋巴瘤(鼻ENKTL)患者鼻部细菌的分布特点与合并鼻部细菌感染的相关指标。方法 34例鼻ENKTL患者取鼻腔分泌物进行细菌培养,同时检测C反应蛋白(CPR)水平、白细胞总数和中性粒细胞百分比等指标。结果鼻ENKTL患者鼻部细菌分布以金黄色葡萄球菌为主(40.00%),其次为凝固酶阴性葡萄球菌(14.00%)、棒状杆菌(10.00%);感染组中鼻部前3位的细菌分别是金黄色葡萄球菌(40.00%)、凝固酶阴性葡萄球菌(13.33%)、变形杆菌(10.00%)。合并鼻部感染时,CPR水平较非感染组显著升高(P<0.01),而白细胞总数及中性粒细胞比率无显著差异(P>0.05)。结论金黄色葡萄球菌是与鼻ENKTL患者鼻部细菌感染最多见的致病菌,CRP可作为鼻ENKTL患者鼻部感染有意义的辅助诊断指标。
Objective To explore the characteristics of bacterial distribution and related parameters of infection in patients with nasal extranodal NK/T cell lymphoma （ nasal ENKTL）. Methods Bacterial cultures of nasal secretion were carried out in 34 patients diagnosed as nasal ENKTL. Meanwhile, serum level of C reactive protein （CRP） , total white blood cells count and neutrophil differential were examined. Results Staphylococcus aureus was the most popular pathogen in nasal infection of patients with nasal ENKTL （40.00%）. Coagnlasc negative Staphylococcus anreus （ 14.00% ） and corynebacteria were next important bacteria （ 10.00% ）. Staphylococcus aureus （ 40.00% ） , coagulase negative Staphylococcus （ 13.33% ） and Proteus （ 10.00% ） were the top three kinds of bacteria in patients with nasal infection. Serum level of CPR was significantly higher in ENKTL patients with nasal infection than that of non -infection group （ P 〈 0.01 ）, but total white blood cells count and neutrephil differential were not significantly different （ P 〉 0.05 ）. Conclusion Staphylococcus aureus is the most common bacteria in nasal infection of patients with nasal ENKTL. Serum level of CRP can be considered as a meaningful parameter for differential diagnosis of nasal infection in patients with nasal ENKTL.
