摘要
目的:为临床治疗胆汁反流性胃炎(Bile-Reflux Gastritis,BRG)选择安全、有效和经济的药物。方法:选择经胃镜及病理证实的胆汁反流性胃炎94例[1],随机分为Ⅰ组(马来酸曲美布汀联合铝碳酸镁片,44例)、Ⅱ组(盐酸伊托必利联合铝碳酸镁片,50例),观察治疗4周前后上腹痛、腹胀、嗳气或胸骨后灼痛、恶心或呕吐等症状以及发作频度、组织学积分的变化,另外胃镜和血液检查在治疗后4周也需要复查。通过计算成本-效果比(C/E)及增量成本-效果比(△C/△E)的值来评价何种治疗方案具有临床选择优越性。结果:Ⅰ、Ⅱ组中总有效率各分别约为89%、88%,差异无统计学意义(P>0.05);不良反应率差异无统计学意义(P>0.05);Ⅰ组的C/E比为2.41,Ⅱ组的C/E比为3.60,△C/△E来看,Ⅱ组要多获得1个单位效果,需多花费85.97元。敏感度分析结果:Ⅰ组的C/E比为2.17,Ⅱ组的C/E比为3.24,△C/△E来看,Ⅱ组要多获得1个单位效果,需比I组多花费86.05元,也证实了原结果的可靠性。结论:马来酸曲美布汀联合铝碳酸镁片组对BRG从临床疗效和经济学的角度来看,优于盐酸伊托必利联合铝碳酸镁片组,临床值得推荐。
Objective To select the drugs which can treat bile-reflux gastritis to be safe、effective and economical.Method Ninety-four patients with BRG who proved by gastroscopy and pathology were randomized divided into group I（Trimebutine Maleate combined with hydrotalcid;44 cases）and II（Itopride Hydrochloride combined with hydrotalcid;50 cases）.Symptoms including abdominal pain、abdominal distension、gas、retrosternal pain or heartburn and nausea or vomiting were investigated about 4 weeks before and after treatment.Gastroscopy and blood test were also reexamined.Results Total effective rate of I group was about 89%,II group was 88%,adverse reaction rate in I and II group was not statistically significant（P0.05）.C/E ratio of I group was 2.41,C/E ratio of II group was 3.60;from △C/△E we can see,if II group wanted another 1 unit-effect,85.97 Yuan should be paid.Sensitivity analysis results：C/E ratio of I group was 2.17,C/E ratio of II group was 3.24;from △C/△E we could see,if II group wanted another 1 unit-effect,86.05 Yuan should be paid.The reliability of the primary results was confirmed.Conclusion Using Trimebutine Maleate combined with hydrotalcid to treat BRG has more efficacy and saving than Itopride Hydrochloride combined with hydrotalcid.This is worth to recommend in clinical.
