摘要
目的 总结应用逆行腓肠神经营养血管岛状皮瓣的经验。方法 1997年 1月～ 1999年 4月以腓肠神经及其滋养血管为蒂的小腿后侧岛状皮瓣 ,修复足跟及足背皮肤软组织缺损 6例 ,足跟部慢性溃疡 2例 ,慢性骨髓炎 2例 ,骨缺损瘢痕挛缩伴瘘管形成 1例。皮瓣切取范围 5 cm× 8cm～ 8cm× 12 cm。结果 术后皮瓣全部成活 ,未发生血管危象。伤口 期愈合。随访 3～ 12个月 ,溃疡、骨髓炎及瘘管无复发 ,皮瓣感觉轻度恢复。结论 此皮瓣不损害重要血管神经 ,供区比较隐蔽。手术方法操作简便。皮瓣血运丰富 ,可有效地修复足跟和足背皮肤软组织缺损的创面 。
Objective To sum up the experience of clinical application of distal base sural island flap. Methods\ From January 1997 to April 1999, the posterior island flap of leg pedicled with sural nerve and its nutrient vessels was applied to repair heel or dorsum of foot in 6 cases, chronic ulcer of heel in 2 cases, chronic osteomyelitis in 2 cases, scar contracture of bone defect accompaning fistulation in 1 case. The range of flap was 5 cm× 8 cm to 8 cm×12 cm.Results\ All the flaps survived completely without vascular crisis. All the wounds healed by first intention. Followed up 3 to 12 months, no ulcer, osteomylitis, fistulation were recurrence, and the sensation of flap was recovered slightly. Conclusion\ The flap do not damage critical blood vessels and nerves, the donating region is hidden. The manual of operation is simple and blood supply of flap is sufficient.It can repair the defect of soft tissue on heel and dorsum with high survival rate.
