肝癌多普勒超声和三维彩色血管能量成像研究(附97例报告)

STUDY ON HEPATIC CARCINOMA WITH DOPPLER ULTRASONOGRAPHY AND THREE DIMENSIONAL COLOR POWER ANGIOGRAPHY

摘要 目的探讨肝癌的血供特点及多普勒超声和三维彩色血管能量成像 (threedimensionalcolorpowerangiography ,3D -CPA)对肝癌的诊断价值。 方法采用ATLHDI - 30 0 0超声仪 ,C4- 2超宽频探头 ,对 5 3例原发性肝癌 (hepatocellularcarcinoma ,HCC)和 44例肝转移癌 (hepaticmetastases,HM )进行彩色多普勒结合脉冲多普勒(pulsedwaveDoppler,PD)检查 ,部分病例行 3D -CPA检查并与血管造影 (angiography ,AG)和病理学检查结果对照。结果HCC与HM均以瘤外弧形血流及瘤内树枝状分支血流为特点 ,其血流速度高 ,阻力指数大 ;且≤ 3cm的肝癌以动脉供血为主 ;>3cm者多为动静脉混合血供 ,二者血供类似 ,无显著性差异 (P >0 .0 5 )。 3D -CPA检查6 6 .0 %的肝癌病例表现为特征性的“众星捧月征” ,此结果与AG及病理检查结果一致。结论 3D -CPA结合血流参数的检测可作为目前评价肝癌血供的首选方法。 ObjectiveTo evaluate the blood supply characteristic of hepatic carcionomas and diagnostic value of Doppler ultrasonography and three dimensional color power angiography (3D-CPA) in hepatic carcinoma.MethodsATL HDI-3000 ultrasonic system and C4-2 super wide frequency transducer were available. The study consisted of 53 cases of hepatocellular carcionomas (HCCs) and 44 cases of hepatic metastases (HMs). For all the cases, color Doppler flow imaging(CDFI), CPA, pulsed Doppler(PD) examination were performed.Contrast study of 3D-CPA with X-ray angiography (AG) and pathology examination were done in some cases.ResultsThere was no significant difference in the blood supply of HCCs and HMs.They were all characterized by radial blood flow in surrounding regions and intratumoral vessels like branches of trees, with high velocity and high resistance index. HCCs that ≤3 cm were supplied by arteries primarily and that >3 cm were supplied by both arteries and veins. 66.0 % HCCs displayed as “a myriad of stars surrounding the moon” by 3D-CPA. The results were corresponded to the result of AG and pathology examination.Conclusion3D-CPA combined with PD can be used as a best method to evaluate the blood supply of hepatic tumors.
