全麻下核心及体表温度的变化 被引量:13

Changes of core and peripheral surface temperatures under general anesthesia

摘要 目的探讨全麻后核心和体表温度的变化及二者的关系,全身耗氧量和产热量变化对核心体温的影响。方法随机选择19例全麻下行择期开颅手术的病人,以鼓膜温度为核心温度,足背和指尖温度为体表温度,臂-指温度差为外周血管舒缩指标,连续观察诱导前、诱导后10min。20min、30min、40min、50min、60min、2h、3h、停药时、停药后10min、20min、30min、40min、50min和60min的各温度变化及吸入麻醉药浓度、耗氧量、产热量变化。结果随着麻醉加深,全身耗氧量、产热量和鼓膜温度显著降低,足背和指尖温度明显升高,臂-指温度差降低由>0℃变为<0℃;麻醉停止后,全身耗氧量、产热量、鼓膜温度和臂一指温度差显著增加,足背和指尖温度显著降低。结论全麻引起核心温度降低,与外周体表温度升高和产热量降低有关;全麻终止后核心体温升高,与外周体表温度降低和产热量增加有关。 Objective To probe into the changes of core and peripheral surface temperatures and the effect of systemic oxygen consumption (SOC) and metabolic heat production (MHP) on core temperatures following general anesthesia. Methods Nineteen patients, scheduled for selective craniotomy under general anesthesia, were randomly selected. Tympanic temperatures were taken as core temperatures, the temperatures in dorsal foot and fingertip as peripheral surface temperatures, and forearm-fingertip temperature gradients as marks of peripheral vasodilation and vasoconstriction. All temperatures, concentrations of inhaled anesthetic, oxygen consumption and metabolic heat production were measured at following points: before induction, 10, 20, 30, 40, 50, 60 min, 2 and 3 h after induction, immediately, 10, 20, 30, 40, 50, 60 min after stopping of inhalation. Results Along with deepenning of anesthesia, SOC, MHP and trmpanic temperature markedly lowered, temperatures in dorsal foot and fingertip obviously rose, and forearm-fingertip temperature gradients decreased from > 0'C to < 0'C. After stopping of inhalation, SOC, MHP, tympanic temperatures and forearm--fingertip gradients greatly increased, and temperatures in dorsal foot and fingertip lowered. Conclusions General anesthesia causes drop of core temperatures relating with increase Of peripheral surface temperatures and decrease of MHP. After termination of anesthesia, core temperatures rises relating with decrease of peripheral surface temperatures and MHP increase.
