摘要
目的探讨脑膜癌病的临床特点。方法回顾性分析25例脑膜癌病患者的临床资料。结果本组中既往有恶性肿瘤病史7例;临床表现为头痛24例,恶心呕吐21例,意识障碍及四肢无力各6例,癫痫发作、精神症状及视力减退各5例等;脑膜刺激征阳性20例,视乳头水肿12例。CSF压力升高13例,蛋白升高16例,糖、氯化物降低14例;23例CSF细胞学检查发现异型细胞,17例CSF细胞免疫组化染色为阳性。头颅MRI平扫均未发现脑膜异常信号,5例头颅MRI增强扫描2例出现脑膜强化。结论脑膜癌病临床表现以颅内压升高症候群为主,CSF细胞学及细胞免疫组化染色可确诊并发现癌细胞的来源。
Objective To discuss the clinical features of meningeal carcinomatosis（MC）.Methods The clinical data of 25 MC patients were analyzed retrospectively.Results In this group,there were 7 cases with malignant tumor history.The clinical manifestations were headache in 24 cases,nausea and vomiting in 21 cases,disturbance of consciousness and limb weakness in 6 cases respectively;seizures,mental symptoms and hypopsia in 5 cases respectively,etc.The positive meningeal irritation was in 20 cases,papilledema was in 12 cases.The CSF pressure was increased in 13 cases,the level of CSF protein was increased in 16 cases,glucose and chlorine were decreased in 14 cases.The tumor cells were found out in 23 cases by CSF cytological examination;while,the immunohistochemical staining was positive in 17 cases.The skull MRI scan were not found meningeal abnormal signal.The meningeal enhancement was found out in 2 of 5 cases by MRI enhanced scan.Conclusions The main clinical manifestation of MC is intracranial hypertension sympotem group.CSF cytological examination and immunohistochemical staining can make diagnosis and fount out the source of cancer cells.
