摘要
目的:探讨自拟加味当归补血汤治疗下肢骨折术后贫血的临床疗效。方法:回顾性分析2010年1月～2011年9月期间,排除了内科疾病和输血患者且资料完整的下肢骨折术后失血性贫血病例68例。根据是否进行术后加味当归补血汤治疗分为治疗组23例和对照组45例。对两组患者基本资料,血红蛋白(Hb)和红细胞计数(RBC)的变化,中医临床证候疗效以及贫血疗效情况进行比较。结果:术后2周,治疗组血液学指标Hb和RBC明显高于对照组(P<0.01);患者中医证候评分中,除"面色萎黄"一项两组无明显差异外(P>0.05),治疗组的"头晕眼花"、"气短"、"神疲乏力"等症状总改善率均优于对照组(P<0.05);治疗组与对照组贫血改善的总有效率分别为91.3%及68.9%,两组比较差异有统计学意义(P<0.05)。结论:运用中医"补血益气、活血祛瘀"理论而来的加味当归补血汤在治疗下肢骨折术后贫血中具有确切的纠正贫血指标、改善气血亏虚症状的作用。
Objective： To research the clinical efficiency of modified Dangguibuxue Decoction therapy for anemia after lower limb fracture surgery. Methods：From January 2010 to September 2011, 68 patients without medical diseases or transfusion got anemia after lower extremity operation. They were included in the retrospective investigation. There were 23 patients in the trial group who received modified Dangguihuxue Decoction therapy, while 45 patients in the control group didn＇t. Their fundamental information, changes of hemoglobin concentration and RBC count, therapeutic effect of Chinese medical clini- cal symptom and curative effect of anemia were compared between the 2 groups. Results.. 2 weeks after operation, both he- moglobin concentration and RBC count in the trial group were obviously higher than that in the control group （P〉0.01）. The traditional Chinese symptoms including ＂dizziness＂, ＂short breath＂, ＂lassitude＂ in the trial group were improved more obviously than that in the control group （P〈0.05） , except ＂complexion etiolate＂ （P〉0.05）. The effective rate of anemia is 91. 3% in the trial group and 68.9% in the control group, and there was significant difference between the groups （P〈 0.05）. Conclusion： Modified Dangguibuxue Decoction therapy had exactly benefit in increasing Hemoglobin and RBC levels and improving the traditional Chinese symptoms when used to treat anemia after lower limb fracture surgery.
