胃间质瘤30例临床诊治分析 被引量：1

摘要 目的探讨胃间质瘤的临床特点、病理免疫组织化学特征及手术预后情况。方法对30例外科手术切除的胃间质瘤进行回顾性分析。结果 21例以上消化道出血为首发症状,上腹部隐痛不适者7例,腹部肿块2例;肿瘤位于胃底18例,胃体9例,胃窦3例;良性间质瘤质地偏硬,交界性及恶性间质瘤大多质地较软,常有出血坏死灶;免疫组化显示CD117阳性28例(93.3%)、CD34阳性23例(76.7%)。本组患者术后随访2～5年,恶性胃间质瘤5例在术后半年内因广泛转移而死亡,3例在术后2年再次出现复发及肝转移,对于肝及胰腺转移者,手术效果较差。结论胃间质瘤临床表现缺乏特异性,CD34及CD117可作为诊断胃间质细胞瘤的特征性免疫标记物;应高度重视初次手术的完全切除,术后密切随访防止复发转移。
