摘要
目的:观察益肾活血方治疗糖尿病肾病Ⅲ～Ⅴ期(气阴两虚血瘀证)的疗效。方法:选取糖尿病肾病Ⅲ～Ⅴ期(气阴两虚血瘀证)患者45例,采用平行对照研究方法,随机分为治疗组23例,对照组22例。两组均采用常规药物治疗,治疗组加用益肾活血方,疗程均为28d。观察患者治疗前后的疗效及各项生化指标。结果:(1)临床综合疗效:治疗组总有效率为82.6%,对照组总有效率为为45.5%,两组间比较差别有统计学意义(P<0.01),治疗组优于对照组。(2)对中医证候的改善作用:治疗组总有效率为95.7%,对照组为68.2%,两组间比较差异有统计学意义(P<0.05),治疗组优于对照组。(3)对疗效性指标的影响:治疗组自身前后对照,各项疗效性指标变化差异均有统计学意义(P<0.01);对照组自身前后对照,HbA1c、Alb、Scr、BUN、eGFR有改善(P<0.05或P<0.01),24h-Upr差异无统计学意义(P>0.05)。治疗后两组疗效性指标改变组间比较,24h-Upr、Alb、Scr、BUN、eGFR的改变差异均有统计学意义(P<0.01),治疗组优于对照组;HbA1c的改变差异无统计学意义(P>0.05)。(4)对安全性指标的影响:两组自身前后对照,各项安全性指标变化差异无统计学意义(P>0.05),治疗后两组安全性指标改变组间比较差异无统计学意义(P>0.05)。结论:益肾活血方治疗糖尿病肾病Ⅲ～Ⅴ期(气阴两虚血瘀证),能改善DN患者的临床症状,减少蛋白尿、降低血肌酐、增高肾小球滤过率;同时无严重不良反应。
Objective ：To observe the effect of Yishen Huoxue decoction on treating diabetic nephropathy Phase Ⅲ～Ⅴ （ Qi - Yin Deficiency and Blood stasis Syndrome）. Methods ：45 cases were randomly divided into treatment groups（23 cases） and control groups （22 cases）. Both groups were given basic medicine, while treatment group was given Yishen Huoxue decoction. Both groups were treated for 28 days. After treatment, the efficacy and clinical indexes were observed and compared. Results：There is remarkable difference at decreasing twenty -four- hour urine protein excretion in The treatment group, and both treatment group and comparison group have showed statistical significance in terms of reduction of HbAlc,Scr,BUN and heightened plasma albumine and eGFR. After treatment, the curative effect index changes of twenty - four - hour urine protein excretion, plasma albumine, Scr, BUN and eGFR showed a significant difference （ P 〈 0.01 ） between treatment group and control group, while the changes of HbA1 c have no significant difference （P 〉 0.05 ）. Conclusion： Yishen Huoxue decoction on the basis of basic therapy can significantly improve the clinical effect of DN Ⅲ～Ⅴ , and alleviate the clinical symptoms, reduce proteinuria and serum creatinine,height eGFR and so on. It is worth to use in clinic on the basis of obviously clinical curative effect and safety.
