连续开大垂直曲强化上颌重度拥挤牙列矢状向支抗的临床研究 被引量:1

摘要 目的探讨连续开大垂直曲在上颌重度拥挤牙列强化矢状向支抗的临床应用。方法选择上颌重度拥挤、减数第一前磨牙的病例20例,随机分为试验组和对照组,每组10例。试验组采用直径0.36 mm(0.014英寸)澳丝弯制连续开大垂直曲,对照组采用上颌Nance弓配合口外弓,研究过程均为初步排齐上颌拥挤牙列,并使上颌尖牙远中移动至紧邻第二前磨牙近中,测量研究过程前后的上颌牙列矢状向支抗稳定性。结果试验组U6M-RL增加(1.34±0.30)mm(t=1.41,P=0.07),对照组U6M-RL增加(1.19±0.20)mm(t=1.36,P=0.09),两组差异无统计学意义(t=1.82,P=0.08)。试验组U6M-APo减少(0.22±0.03)mm(t=0.91,P=0.15),对照组U6M-APo减少(0.18±0.02)mm(t=0.88,P=0.19),两组差异无统计学意义(t=1.53,P=0.11)。结论连续开大垂直曲远中移动尖牙,初步排齐上颌牙列,较好地强化了上牙列矢状向支抗。 Objective To evaluate the effects of continuous open vertical loops on reinforcing sagittal anchorage in the stage of alignment for severe crowding dentition.Methods Twenty patients with maxillary severe crowding were divided into two groups,underwent continuous open vertical loops and headgear anchorage combined with Nance arch to reinforce molar anchorage in the stage of alignment.Cephalometric analysis was performed to evaluate the movement of maxillary first molars before and after alignmentation.Results U6M-RL increased(1.34±0.30) mm(t=1.41,P=0.07) and(1.19±0.20) mm(t=1.36,P=0.09),U6M-APo reduced(0.22±0.03) mm(t=0.91,P=0.15) and(0.18±0.02) mm(t=0.88,P=0.19) in two groups respectively.There was no significant differences between two groups(P0.05).Conclusion Continuous open vertical loops can reinforce sagittal anchorage as aligning teeth with severe crowding.
