髌股关节退行性病变的摄影方法学研究

摘要 目的探讨髌股关节多角度侧位投照方法的临床价值。方法收集50例膝关节疼痛的患者,分别采取180°、135°、90°膝关节屈伸角度侧位等多角度髌股关节X线摄影,比较髌股关节退行性病变的X线表现。结果多角度髌股关节侧位投照显示,髌骨后缘关节面增生毛糙50例,股骨滑车面增生硬化及髌股关节间隙变窄43例,胫股关节间隙变窄23例,髌骨固定6例,而常规位置阳性显示率较低。2种投照摄影方法比较,股骨滑车、髌骨后缘、髌股关节间隙及髌骨活动范围等部位显示率差异有统计学意义(P<0.05)。多角度投照方法对观察股骨滑车面、髌骨关节面、髌骨移动范围、髌股和胫股关节间隙以及确定髌骨高位等退变特征有明显优势。结论多角度髌股关节侧位投照是一种相对客观的显示髌骨位置及髌股关节间隙的X线摄影方法,可在临床检查中广泛使用。 Objective To explore patellofemoral joint angles as a side for the clinical value of method. Methods Collect 50 cases of the knee joint ache patients. The multiple perspectives patellofemoral joint side a X-ray photography, to observate patellofemoral joint degenerative diseases of X line performance. Patients in photography before bed stand sideways, take the knee flexion inspected respectively for side of 180 ° , 135 ° , 90 ° ,and absorb double eontralateral patellofemoral joint were compared. Results Get 300 votes different angles side a knee X-ray patellofemoral joint space, femoral pulley surface and the patellar rear edge display rate is 100%. After statisticsx2, multi-angle patellofemoral joint side a X-ray photography in different aspects, and gender that there is no difference between rate (/9〉0.05). After the joint laxity margin de- generation first and then femoral pulley surface hyperplasia, patellofemoral joint gap narrowed, tibial strands of joint degenerative diseases and knee flexion, knees and holds the limited scope of patella activities or fixed. Conclusion Patella joints angles as a side vote is a relatively objec- tive display position and the corresponding patella joint gap between the X-ray photography.
