实时三平面定量组织速度成像技术评价冠心病患者心室舒张同步运动 被引量:4

Evaluating ventricular diastolic synchrony in patients with coronary heart disease by real-time 3-plane quantitative tissue velocity imaging

摘要 目的探讨实时三平面定量组织速度成像技术对评价冠心病患者左右心室壁舒张同步运动的临床价值。方法同步实时采集行冠脉造影139例患者(分四组)心尖四腔观、心尖两腔观和心尖左室长轴观的组织速度动态图像,测量左右心室壁基底段舒张早期达峰时间(Te)并计算室内及室间舒张不同步差量进行对比研究。结果部分左室壁基底段Te在组间比较中有统计学差异(P<0.05),室内及室间舒张不同步差量的组间比较无统计学差异(P>0.05),左室下壁Te对冠状动脉狭窄≥50%的诊断价值最高(P<0.01)。结论实时三平面定量组织速度成像技术可较准确评价心室壁舒张同步运动,对诊断缺血心肌的敏感性高于常规超声心动图。 Objective To investigate the clinical value of real-time 3 plane quantitative tissue velocity imaging in ventricle diastolic synchrony of coronary heart disease (CHD). Methods We selected of 139 cases proved by coronary angiography through synchronous real-time dynamic image acquisition of *issue velocity imaging, which included apical 4 chamber view, apical 2-chamber view and long axis of left ventricle (LV) view, then studyied these parameters such as time of early dias- tolic velocity (Te) in the left and right ventricular walls~ basal segments, intraventricular and interventricular diastolic dyssynchrony. Results Te indicates significant difference in several LV wails' basal segments, P d0.05. Intraventricular and interventricular diastolic dyssynchrony have no significant difference, P ~〉0.05. LV inferior wall Te has maximum di- agnostic value on coronary artery stenosis in the conditions of vessel 〉 50%, P d0.01. Conclusion The real time 3-plane quantitative tissue velocity imaging used in evaluating ventricle diastolic synchrony has higher sensitivity than conventional echocardiography in the diagnosis of myocardial ischemia.
