摘要
目的探讨ScvO2在外科术后危重患者中的临床应用价值.方法选择入SICU的外科术后危重病患者共18名,根据预后及有无并发MODS分为对照组、MODS组以及死亡组3组,进行持续ScvO2及常规血流动力学监测,并进行相关性分析.结果 3组患者ScvO2值在早期差异有统计学意义(P<0.05),但患者的心率、血压、CVP监测结果直到经抢救治疗后24 h才显示出差别.ScvO2监测值与血乳酸值、APACHEⅡ评分存在负相关(r:-0.721/-0.80).结论患者ScvO2水平与病情严重程度相关,早期持续监测对评价病情和判断预后有重要意义.
Objective To study the clinical value of continuous central venous blood oxygen saturation(ScvO2) in post-operative critical patients.Methods Eighteen post-operative critical patients from SICU were involved and divided into three groups:the control group,the MODS group and the death group,continuous ScvO2 and conventional hemodynamic monitoring in different periods of disease were compared,and the correlation analysis between the ScvO2,lactic acid and APACHEⅡ were undertaken.Results A significant difference were found in the ScvO2 from patients of all groups in early stage,but the patients' heart rate,blood pressure,CVP monitoring results showed difference till 24 hours later in SICU.There was a linear correlation between ScvO2 and blood lactic acid,the scole of APACHEⅡ(r:-0.721/-0.80).Conclusion ScvO2 level is related to the disease severity of post-operative critical patients,continuous monitoring in early stage has important significance for evaluation of the condition and prognosis.
