摘要
目的评价CT(平扫、增强、CT胆囊造影)对胆囊息肉样病变及良、恶性的诊断价值。方法回顾性分析50例术前均经CT(平扫、平扫+增强、CT造影+增强)、彩超诊断并经手术病理证实的胆囊息肉样病变,包括胆固醇性息肉、炎性息肉、厚壁性胆囊炎、胆囊腺瘤、胆囊腺癌、小胆囊癌。结果经手术病理证实的50例胆囊息肉样病变中,良性38例、恶性12例,CT及彩超对胆囊息肉样病变都做出了诊断。在对良、恶性鉴别上,CT诊断出恶性11例,占91.7%,彩超诊断出8例,占66.7%。对38例良性息肉性病变的诊断诊断上,CT和彩超对38例良性息肉样病变都符合病理诊断;在病理分型上,CT诊断符合病理分型27例,占71.1%,彩超诊断符合病理分型34例,占89.5%。结论 CT扫描(平扫、平扫+增强、CT造影+增强)和彩超对胆囊息肉样病变的判定都比较准确。但在良、恶性的鉴别上CT扫描优于彩超,而彩超对胆囊息肉样病变的良性息肉样病变的病理分型较好。
Objective To evaluate the diagnostic value of CT in benign and malignant polypoid lesions of gallbladder.Methods Fifty cases of polypoid lesions of the gallbladder diagnosed by CT（plain scan,plain scan and enhancement,CT arteriography and enhancement） and color Doppler ultrasound and proved by operation and pathology were retrospectively analyzed.The scope included cholesterol polypus,inflammatory polypus,thick-wall cholecystitis,gallbladder adenoma,adenocarcinoma of the gallbladder and carcinoma of gallbladder.Results Among the 50 cases,38 cases were benign,12 cases were malignant.Polypoid lesions of the gallbladder can be diagnosed by both CT and color Doppler ultrasound.Differential diagnosis of benign and malignant polypoid lesions of the gallbladder as below,11 cases of malignant were diagnosed by CT,accounted for 91.7%,8 cases were diagnosed by color Doppler ultrasound,accounted for 66.7%.CT and color Doppler ultrasound were coincident with pathological diagnosis.Pathological types as follow,27 cases were coincident with CT diagnosis,accounted for 71.1%,34 cases were coincident with color Doppler ultrasound diagnosis,accounted for 89.5%.Conclusion CT scan（plain scan,plain scan and enhancement,CT arteriography and enhancement） and color Doppler ultrasound can accurately diagnose polypoid lesions of the gallbladder,but CT is better than color Doppler ultrasound in the differential diagnose of benign and malignant polypoid lesions of the gallbladder.However,color Doppler ultrasound is superior to CT scan in distinguishing benign lesion of polypoid lesions of the gallbladder.
