摘要
目的研究乳果糖联合枯草杆菌二联活菌肠溶胶囊治疗肝硬化所致肝性脑病的治疗效果。方法将2009年1月~2012年1月收治的72例诊断为1、2期肝性脑病患者随机分为观察组与对照组,对照组给予低蛋白饮食、保肝、调整芳香氨基酸与支链氨基酸比例、纠正电解质紊乱等综合治疗,观察组在对照组综合治疗的基础上加用乳果糖(每日30~100mL)、枯草杆菌二联活菌肠溶胶囊(2片,日3次)口服,治疗4周,观察患者肝性脑病的改善以及血氨、肝功能的改变。结果观察组及对照组治疗总有效率分别为83.3%(30/36)和55.6%(20/36),差异有统计学意义(χ2=6.545,P=0.011);观察组血氨下降更显著(P〈0.01);与对照组比较,观察组对肝功能的影响无明显优越性(P〉0.05)。结论乳果糖联合枯草杆菌二联活菌肠溶胶囊治疗肝硬化所致肝性脑病效果好。
Objective To explore the effect of lactulose combined with live combined bacillus subtilis and enterococcus faecium enteric-coated capsules (Medlic-S) in the treatment of patients with mild hepatic encephalopathy after hepatic cirrhosis. Methods Seventy-two patients with 1 or 2-grade hepatic encephalopathy after hepatic cirrhosis from January 2009 to January 2012 were selected. They were randomly divided into observation group (n = 36) and control group (n = 36). The control group received routine comprehensive treatment include low protein diet, liver protection, regulate the proportion of aromatic amino acid and branched-chain amino acid, retrieve electrolyte disturbances, the observation group were administered orally with Lactulose 30-100 mL/d and Medlic-S 2 pills, 3 times a day for 4 weeks in addition to the basis of the routine comprehensive treatment. The improvement of hepatic encephalopathy and the changes of liver function and blood ammonia were observed. Results The total effective rate of observation group and control group were 83.3% (30/36) and 55.6% (20/36) respectively, the observation group were significantly improved (χ2=6.545, P = 0.011). Compared with the control group, the blood ammonia of the observation group were decreased effectively (P 0.01), however, no significant difference of the liver function (include AST, ALT and TBIL) was found in the study above (all P 0.05). Conclusion Lactulose and live combined bacillus subtilis and enterococcus faecium enteric-coated capsules in treating mild hepatic encephalopathy has better efficacy.
