鼻腔鼻窦畸胎癌肉瘤4例临床病理分析 被引量：19

摘要 目的探讨鼻腔鼻窦畸胎癌肉瘤(sinonsal teratocarci-nosarcoma,SNTCS)的临床病理特点、诊断、鉴别诊断及预后。方法采用免疫组化EnVision两步法对4例SNTCS进行染色并复习相关文献。结果 4例患者中3例为男性,1例为女性;1例并发颅脑及肝脏转移,1例术后半年复发。SNTCS镜下形态复杂多样,可见未成熟的鳞状细胞巢及腺样结构,部分腺体异型,梭形肉瘤样区围绕不成熟的鳞状细胞巢及腺癌成分分布,有些病例可见嗅神经母细胞瘤样区域及神经原纤维基质,肿瘤间质中尚能见到未分化/原始的肿瘤细胞及分化不等的骨及软骨组织。结论 SNTCS是一种罕见的恶性肿瘤,可能为原始胚胎组织或发育过程中迷离在上呼吸道的多潜能干细胞引起的一种病变。其组织学成分复杂,形态多样,取材不充分可能导致误诊,特征性的组织病理学和免疫表型有助于诊断及鉴别诊断。
