摘要
目的建立以我院为中心的导管维护网,为带管出院的患者提供延续性护理并提高导管的维护质量。方法对我院周边的三级乙等医院或县市级医院进行导管维护理论及技能的培训,建立长期合作的导管维护网,为带管出院回当地的患者提供规范的导管维护及并发症处理,实现顺畅的转诊流程,使留置PICC等导管的患者获得全程的照护。结果我院的PICC置管数,从2006年的720例上升至2011年的1702例,其中2011年PICC置管例数较2010年增加了25%,门诊维护人数较2010年下降了6%;患者在出院期间的导管相关并发症明显下降,其中导管意外滑脱的发生率下降了6.2%,导管堵塞的发生率下降了8.3%,局部感染的发生率下降了6.6%。结论导管维护网可使患者享受规范便捷的导管护理服务,降低因维护不当导致的并发症,确保患者带管期间的安全,通过定期的培训和交流,持续提升了当地医院导管维护质量,达到医院和患者双赢的效果。
Objective To establish a third-grade class-A hospital centered network for the management of central venous catheters, so as to provide continuous high-quality catheter maintenance for discharged patients with central venous catheters. Methods The nurses in class-B hospitals, municipal hospitals or country hospitals near class-A hospitals were trained about catheter maintenance skills. Then, they provided standardized catheter maintenance and management of the catheter related complications for discharged patients. Moreover, a dual referral system and long term cooperation network was established to provide patients with continuous services. Results After the establishment of catheter maintenance network, the number of PICC insertion increased from 720 cases in 2006 to 1702 cases in 2011. The number of PICC insertion increased by 25%, and the number of outpatient catheter maintenance decreased by 6% in 2011 compared with in 2010. The incidence of catheter-related complications significantly decreased. The incidence of catheter dislodgment,catheter occlusion, and local infection decreased by 6.2%,8.3% and 6.6%, respectively. Conclusions Establishment of third-grade class-A hospital centered catheter maintenance network can provide patients with standardized and convenient catheter maintenance service, decrease the incidence of catheter-related complications and ensure patient safety. Moreover, regular training and exchange between networks can improve the quality of nursing, and achieve benefit both for patients and hospitals.
