大脑中动脉分叉部动脉瘤分型与分支管径关系的研究 被引量:5

Research of relationship between types of middle cerebral artery bifurcation aneurysms and diameters of two daughter arteries

摘要 目的探讨大脑中动脉(MCA)分叉部动脉瘤的分型与两分支管径的关系。方法以14例偏侧型动脉瘤和8例经典型动脉瘤为研究对象,通过3D-DSA测量患侧MCA两支干起始处的管径并计算其支干管径比,对两组动脉瘤的支干管径比进行两独立样本t检验。结果偏侧型和经典型两组动脉瘤的支干管径比具有统计学差异(P<0.05)。结论对于双干型MCA,两分支管径相差较小的多发生经典型动脉瘤,相差较大的多发生偏侧型动脉瘤。 Objective To investigate the relationship between the type of middle cerebral artery (MCA) bifurcation aneurysms and the diameters of the two daughter arteries of MCA. Methods Twenty-two MCA bifurcation aneurysms were traditionally classified into classical neck type (n=8) and deviating neck type (n=14) according to the neck location. The diameters of the MCA daughter arteries were measured by 3-dimensional digital subtraction angiography (3D-DSA) and the ratios of the diameters of two daughter arteries of MCA were calculated in 22 patients with MCA bifurcation aneurysms. The relationship of the types of MCA bifurcation aneurysms with the ratios of the diameters of the MCA two daughter arteries was analyzed. Results The diameter ratios of the MCA two daughter arteries in the patients with deviating neck type MCA aneurysms and ones with classical neck type MCA aneurysms were 1.42_+0.29 and 1.16_+ 0.15 respectively. There was significantly difference in the diameter ratios of the MCA daughter arteries between the patients with deviating neck type aneurysms and ones with classical neck type aneurysms (P〈0.05). Conclusions The classical neck type MCA bifurcation aneurysms are more common in the patients with lower ratios of the MCA daughter arteries diameters than those in the patients with higher ratios of their diameters, in whom the deviating neck type MCA bifurcation aneurysms are more common in the patients with lower ratios of their diameters.
