摘要
目的提高慢性阻塞性肺病(COPD)患者应用无创正压通气(NIPPV)治疗时的舒适度。方法成立专门的护理干预小组,用循证的方法对75例确诊为COPD并使用NIPPV治疗的患者采取有效的护理措施,包括呼吸机同步护理,面部皮肤受压的护理,口、咽部干燥的护理,胃肠胀气护理,心理不适应的护理等。结果采用Brown-Saltzman K评价标准对干预患者进行评价,患者舒适度明显提高,干预小组工作有显著成效。结论应用NIPPV治疗的COPD患者不舒适是其现存的主要护理问题,应给予关注和重视并采取有效的护理措施,才能减轻患者痛苦,达到患者舒适的最终目的。
Objective To improve the therapeutic comfort of non- invasive positive pres- sure ventilation（NIPPV） in patients with chronic obstructive pulmonary disease （COPD）. Methods A special nursing intervention team was established to implement effective nursing measures for 75 COPD patients with NIPPV under guidance of evidence based method, and the measures were composed of synchronous ventilator nursing, nursing for compression on faces, nursing for mouth and pharynx dryness, nursing for flatulence and psychological nursing. Results Intervention as- sessment were based on Brown - Saltzman K evaluation criteria. Comfort degree of patients im- proved significantly and work of intervention team showed significant efficacy. Conclusion The therapeutic discomfort of COPD patients underwent NIPPV treatment is the main problem of nursing. Much more attention, concern and effective measures should be given in order to relieve sufferings of patients and achieve the ultimate purpose of patients＇ comforts.
