摘要
目的 :对小切口宫体韧带集束断扎筋膜内子宫全切术进行术式探讨。方法 :将同期具有子宫全切术适应症的子宫良性病变患者 10 3例随机分组 ,分别采取经腹小切口宫体韧带集束断扎筋膜内子宫全切术 72例 (研究组 ) ,用常规方法子宫全切术 31例 (对照组 ) ,进行临床资料对比分析。结果 :研究组切口控制在≤ 6 cm ,术式简化安全 ,在手术时间、出血量、术后肠排气时间、术后病率、术后住院时间均明显优于对照组 (P <0 .0 1)。差别具有显著意义。结论 :经腹小切口宫体韧带集束断扎筋膜内子宫全切术解决了常规方法子宫全切术的术式弊瑞 ,保持了盆底组织及阴道解剖上的完整性 ,保证盆底血供 ,将手术创伤降到最低限度。具有切口小、术程短、出血少、康复快 ,符合患者心理需求 ,手术步骤简化 ,安全易掌握 ,实用性强的特点。
Objective:To investigate the effect of internal fascia hysterectomy of abdominal small incision by cluster cutting off and fastening the uterine ligament Method:103 benign uterine affection patients who has hysterectomy indication were allocated randomly,of which 72 patients(the research group) were treated with the new method“the internal fascia hysterectomy of abdominal small incision by cluster cutting off and fastening the uterine ligament”,of which 31 patients(the control group) were treated with the traditional hysterectomy method Result:the incision of the research group is controled ≤6cm Its operation method is simple and safe Its operative time,bleeding amount,postoperative intestinal exhaust time,postoperative morbidity,and postoperative hospitalization time are obviously better than the control group's(P<0 01) respectively Conclusion:The new operation method keeps the integrality of the pelvic floor tissue and vaginal anatomy,assures the vascularization of the pelvic floor,lessens injuries to the minimum,hence showing the following advantages:small in incision,short in operation time,less bleeding,sooner recovery,meets the psychological demand of patients,simple in operative procedure,easy to grasp,and practical etc
