摘要
目的:研究皮肌炎(DM)患者的心脏损害。方法:应用心电图、多普勒超声心动图对34例皮肌炎患者进行检查。结果:发现17.6%出现心电图异常,以窦速和束支传导阻滞多见;个别患者出现心包损害,二尖瓣返流,IVS增厚,舒张功能指标E、A、E/A、IRT、E_(dc)、1/2E_(dc)T、EFF、PFR、RFI异常,收缩功能指标CO升高。结论:DM伴有心肌损害,造成舒张性功能异常,部分收缩功能指标增高可能与心外因素有关。心包损害仅见于少数患者,瓣膜无侵害,返流为继发性改变。
Objective:To study the cardiac damage in patients with dermatomyositis (DM). Methods: Thirty-four patients with DM were examined through electrocardiogram and doppler echocardiography. Results: It showed that the electrocardiogram abnormality rate was 17.6% ,especially sinus tachycardial and bundle block occurred commonly. The pericardial damage and mitral regurgitation appeared uncommonly. Interventricular septum thickened slightly. The parameters of cardiac diastolic function, such as E, A, E/A, IRT, Edc, 1 /2Edc T, EFF, PFR and RFI were abnormal. The systolic index CD level was higher than control group. Conclusion:The DM patients had myocardial damage which induced abnormal diastolic function,but the increased CD level was related with other non-cardiac factors. The pericardium involvement appeared uncommonly and the mitral regurgitation was a secondary change, whereas valve itself was not damaged.
