摘要
目的观察脑外伤患者术中低血压的发生及患者的预后转归,分析影响低血压患者预后转归的临床因素。方法回顾性分析212例行急诊开颅术的脑外伤患者临床资料,将术中出现低血压的患者纳入观察组(44例),未出现低血压的患者为对照组(168例),比较两组患者的预后转归。分析平均动脉压下降幅度、低血压持续时间以及是否合并低氧血症对预后转归的影响。结果观察组重残率、病死率显著高于对照组(P<0.05),提示术中低血压影响患者的预后转归。分析观察组中死亡患者的影响因素,结果提示术中血压下降幅度越大、低血压持续时间越长以及合并低氧血症的患者病死率较高。结论在脑外伤患者的手术过程中应保持脑灌注压,尽量缩短低血压持续时间,并积极纠正低氧血症,在一定程度上降低病死率,改善预后。
Objective To investigate the impact of hypotension during craniotomy on the prognosis of patients with craniocerebral injury underwent operation and analysis its influence factors.Methods A retrospective review was conducted in 212 cases with craniocerebral injury.44 patients who has hypotension during craniotomy was taken into observation group while the rest patients was taken into control group.Comparison was made between two groups in respects of MAP,duration time of hypotension and whether combined hypoxemia.Results The rate of severe disability and mortality in observation group was higher than control group（P0.05）.Analysis was made in these dead cases with different MAP,duration time of hypotension.And the results show that the mortality was increased with the decrease of MAP,or the prolong time of hypotension,or combined hypoxemia.Conclusion The mortality rate is significantly increased in craniocerebral injury patients with hypotension during craniotomy.The prognosis can be improved by preventing low cerebral perfusion pressure,shortening the duration time of hypotension and correction of hypoxemia.
