超声乳化术和不同粘弹剂对角膜内皮细胞影响 被引量:16

The study on the effect of phacoemulsification and different viscoelastic substances on corneal endothelial cells

摘要 研究超声乳化过程中 ,应用不同粘弹剂、角膜内皮细胞形态学改变及细胞损失率的差别 ,明确不同粘弹剂对角膜内皮细胞保护作用的区别。方法 :对 2 0 0例老年性白内障患者施超声乳化术 ,观察使用 Healon、 Healon- GV、甲基纤维素、爱维四种粘弹剂 ,术后 0、 4、 8、 2 4、 48、 72小时角膜内皮细胞形态学及细胞损失率的差别。结果 :Healon、Healon- GV、甲基纤维素、爱维组的细胞损失率分别为 3.6 18%、 1.338%、 8.0 78%、 6 .10 4%。Healon、Healo- GV组与甲基纤维素、爱维组有显著性差异 (P<0 .0 1) Healon、 Healon- GV组间亦有显著性差异 (P<0 .0 1)后两组间差异无显著性 (P>0 .0 5 ) ,各组角膜内皮细胞形态学均有较明显改变。结论 :超声乳化术引起角膜内皮具有临床意义的丢失 ,四种粘弹剂对角膜内皮的保护作用有差别 。 Objective:To study the morphological changes of corneal endothelial cells and the difference of cell loss rates with the use of different viscoelastic substances in the process of phacoemulsfication,and to determine the difference anong protective effect of different viscoelastic substances on corneal endopithelial cells.Methord:phacoemulsification was used to 200 cases of senile cataract and the morphological changes of endothelial cells and the difference of cell loss ratas were observed 0,4,8,24,48,72 hours after the operation with four viscoelastic substances of Healon,Healon GV,HPMC and IVIZ.Results:the cell loss rates of Healon,Healon GV,HPMC and IVIZ are 3 618%,1 338%,8 078%,and 6 104% respectively.There is a significant difference between the groups of Healon and Healon GV and the groups of HPMC and IVIZ(P<0 01).There is also a significant difference between the groups of Healon and healon GV(P<0 01),and there is no significant difference between the latter two groups(P>0 05).Each group has obvious morphological changes of coneal endothelial cells.Conclusion:phacoemulsification causes cell loss of coneal endothelial cells,which is of clinical importance.There is differences among the four viscoelastic substances.Therefore,the viscoelastic cubstances should be chosen acording to the specific clinical conditions.
