摘要
目的 探讨胸腰椎结核经前路病灶清除同时椎间植骨的疗效。方法 对 48例胸腰椎结核采用经前路一次性两侧病灶清除同时施行椎间植骨术作一回顾性分析。结果 无手术死亡和严重并发症 ,其中 2 5例 3～ 6个月摄X线骨像 ,见骨病灶稳定 ,植骨片位置良好 ,少量骨痂形成 ;18例 1a后X线片 ,植入骨片与植骨床椎体融合 ,主观感觉和临床检查均符合骨结核病的康复标准 ,13例并发截瘫者 ,完全恢复 10例 ,部分恢复 3例。结论 选择合适之胸腰椎结核病例 ,施行前路一次性两侧病灶清除并椎间植骨治疗完全可能 ,效果优良 ,不但能清除病灶 ,促进早期康复 ,而且恢复脊柱稳定。
Objective\ To evaluate the curative effects of eliminating latibulum and implanting bone by anterior path at one time.Methods\ Operations by anterior path to eliminate bilateral latibulum and graft bone at one time were performed on 48 patients with thoracic and lumbar spine tuberculosis.Results\ The follow-up results showed that none of the cases died of the operation and occurred serious complications.Postoperative 3～6 months,25 cases X-ray indicated that the latibula were stable,the position of grafted bones were good and some osteotylus had formed.After one year 18 cases,X-ray indicated that the graftad bone had achieved bony union accorded with healing standard of turbercular bone in both subjective feeling and clinical examination.Among 13 paraplegic cases,10 were completely recovered,3 were partially rehabilitated.Conclusion\ It is possible and effective that anterior path treat thoracic and lumbar spine tuberculosis by eliminating bilateral latibula and implatning bone at one time.It can not only eliminate latibulum but also accelerate rehabilitation and promote stability of the spine.
