摘要
健康管理是指导公众进行健康管控的重要环节,它帮助、指导人们成功有效地把握与维护自身的健康。社区卫生服务中心作为实施卫生服务的主体之一,把健康管理的理念贯穿到卫生服务全过程中,既是现代医学模式发展的需要,也是提升自身服务水平、履行社会责任的要求。本研究基于社区卫生服务的实践,对社区健康管理家园的建立与运作进行初步探讨。
Health management is an important link for guiding public to carry the health management,and it helps and guides people to successfully and effectively maintain their own health.Community health services center,as one of main body of implementation of health services,needs to permeate the health management idea throughout the whole process of health services.This is the demand for development of modern medicine mode,and also the requirement to raise the self-service level and to fulfill the social responsibility.In this paper,we primarily discuss the establishment and operation of community health management home based on practice of community health services.
